Three Children's records were randomly selected for review and the following was noted: 1) One child did not have evidence of a current face sheet, first aid/emergency form, transportation form or developmental history form as required by regulation.
Chipping paint was observed in the "Zebra" (Pre-K) classroom, creating a potential hazard to children.

Not all equipment was observed to be secured in the Preschool "Panda" room, creating a potential hazard.
During a review of the playground spaces, the following was learned: the garden hose was left on the playground, creating a potential tripping hazard. The link chains, used to secure the sandbox cover, was observed to be rusty, creating a potential hazard.
