Children’s files were reviewed. There were two files that did not have lead screening. Four files need updated medial physicals. Additionally, one file needs an updated allergy action plan and the parent permission form to admitted medication is required. Five files need the annual update for the enrollment packets. One file had a blank general permission. There were multiple files that did not indicate the date of admission, age of admission, and physical description on the first page of the enrollment packet.
