Children's Records
Children’s files were reviewed. One file was missing the physical description on first page of enrollment packet. Multiple files are due for the annual update of the enrollment packet. Two files need updated physicals and lead screening. One file was missing the date and age at time of admission on the first page of the enrollment packet.
There was access to plastic bags underneath the diapering table due to opened diaper bags. It was observed within the outdoor space there are three climbing structures that do not meet the 6-foot distance fall zones from each side of each structure. This includes the slide that was on the grass, which is not approved.

This monitoring visit inspection recorded no violations or advisories.
One of the kitchen drawer safety latch was not latching and this gives access to sharp knives.

Children’s records were reviewed. One child did not have parent permission to go off premises. Educator does take children for neighborhood walks.
Stairways to basement exit does not have railing.
Educator has two climbing structures. One of the structures (red slide) does not have the 6 foot clearance from all sides.
Educator does not have valid CPR and First Aid certification. Educator completed an online only CPR and First Aid certification, which EEC does not accept. Educator was informed of CPR and First Aid policy. Educator will need to send licensor proof of registration for CPR and First Aid by 2/13/2022.

Educator did not have progress reports accessible. Educator has not been documenting progress reports at the frequency required, this was reviewed with the educator.
There were two outlets that were not covered. There was access to Clorox wipes under kitchen sink in basement level. There was access to tape with sharp edge blade in one of the kitchen drawers in the basement level. It was observed that the heater baseboards had wide openings that allows children to place their hands into the baseboard. Baseboards are not in use, educator indicated they are going to be removed; until then educator must prevent access to the openings on all baseboards.
Two toy shelves and diapering table were observed to be easily moveable.

The educator has three climbing structures that require appropriate ground surfacing and fall zones. The play area was observed to not have the required depth for the ground surfacing material and the ground surfacing does not cover the entire fall zones. The educator has wood chips of which some were observed to be smaller pieces which are a choking hazard. It was observed that two of the structures did not have the 6 foot distance required from each side. Structures cannot be utilized until educator has appropriate ground surfacing and fall zone requirements.
