Two Staff records were randomly selected for review, and the following was learned: 1) Educator A did not have written documentation of 2 professional references as required 2) Educator B (Start date 3/2024) did not have written documented evidence of proving the program with a physical exam as required 3) Educator B did not have written documented evidence of qualifications as required 4) Educator B did not have written documented evidence of current staff observations or Professional Development hours as required by regulation.
