During a review of the indoor space, the following was learned: 1) An educator's purse was in the reach of the children 2) the emergency medication was left in the reach of children when the bag was left on the table 3) a hot coffee cup was left in the reach of children, posing a potential hazard to children.

Child A had an allergy plan that states the child should be given [REDACTED]" as part of their plan, there was no evidence of Benadryl on site at the program.

While the emergency medication was available, it was left in the reach of children when the emergency backpack that stores the medication was left unattended on the picnic table outside, creating a potential hazard to children.

2 staff records were randomly reviewed during the visit and the following was learned: 1) Educator A did not have evidence of 2 professional references as required and did not have evidence of a physical within one year of their hire date as required 2) Educator B did not have evidence of 2 professional references, an application or date of interview as required. Additionally, Educator B did not have evidence of a completed Strong Start 2.0 Certification or an EEC PQR registry.

During a review of 2 randomly selected staff records, the following was learned: 1) Educator A (start date 6/24/2022) did not have evidence of a current staff evaluation or EEC registry as required.

During the visit, it was observed that not all children's belongings were stored in individual storage bins as required by regulation.

2 staff records were randomly selected for review and the following was learned: 1) Educator A did not have evidence of 2 professional references as required, an annual staff evaluation as required; and documentation of a completed program orientation. 2) Educator B did not have evidence of their interview date, 2 professional references, First Aid Certification (start date 5/2/2021) or a completed program orientation as required.

Individual Health Care Plan (IHCP) - R Apr 2025
During the visit, the program did not follow EEC required School-aged ratios.
A child with an IHCP plan has a doctor’s order for “Benadryl” but had the store brand “children’s allergy” on site during the visit.
[REDACTED] were missing all EEC required information including “who trained the educator” and who was “trained” in the [REDACTED]
1)Child A had a medication consent form for [REDACTED] to be given with an elevated fever but there was no evidence of [REDACTED] on site at the program as required. 2) Child B had an [REDACTED] on site that had a prescription that expired 7/28/22.
Child B had an IHCP plan and a medication form that were not signed and dated by their parent as required.
