Children were observed to walk into the storage closet to find spoons snack, children were observed to sit on the gym floor to eat, surfacing was not washed/disinfected.
The emergency information posted has not been updated, removing staff that no longer work in the program as required.
Children's belongings were observed to be stored throughout the program on the floor, not in individual storage bins as required. An educator's bag was observed to be stored on the program floor, accessible to children.
5 IHCP plans were reviewed during the visit the following was learned: *5 IHCP plans were missing all required information, including "who trained" the educator in the child's chronic condition and "who is trained" in the child's condition. *2 IHCP plans were not signed by the doctor as required. *1 IHCP was missing additional information, including the consequences if the plan was not followed and he symptoms to be looking for as required by regulation.
At that time of the visit it was learned that the [REDACTED]has not been conducting fire evacuation drills as required.

[REDACTED] children’s records were randomly reviewed and the following was learned: [REDACTED] children did not have evidence of a progress report from the 2018-2019 school year. One child did not have evidence of their date of admission.
There was no evidence of fire drills having been completed in April, May or June 2019 as required by regulation.

The Site coordinator did not have a handle on the group and was not able to gain control of group. When children challenged them, they stated to children that they were "in trouble" or that they would "call their mom" Two children were observed to ask more than once if they could go outside today and the Site Coordinator was observed to tell them no, the group was "still in trouble from last time."
On the day of the visit, there was no evidence of any kind of curriculum present. Children were observed using/watching children on a device, playing with gym mats or using a single ball together. No activities were present as required.
Children were observed to eat snack on the floor or while walking around the gym space, creating a potential hazard.

2 staff records were reviewed and found to be incomplete. The program was cited for this in October 2021. Educator A did not have evidence of a staff form, references, PQ registry, a physical, MMR records or "Strong Start" trainings as required.
The program did not report to EEC a change in the site coordinator as required by regulation.
During a review of children's records, it was learned that progress reports were not documented for the 2021-2022 school year. The program was cited for this in October 2021.
The program did not have written evidence of having conducted monthly fire drills during the 2021-2022 school year. The program was cited for this during the October 2021 visit. The program did not have evidence of logging the incidents/injuries for the 2021-2022 school year as required. A current EEC License wasn't posted or stored in the program's "licensing binder" that is required to be available to parents as required. The location of the health care policy was not posted or stored in the licensing binder as required.
Children were not observed to wash their hands prior to eating snack. Children were observed to walk through the program eating apple sauce or sitting on the floor to have snack. There was not an appropriate place for children to sit and eat as required.
