A review of three staff record’s indicated: - Two staff did not have evidence of a current physical examination.
In Preschool Classroom #4, there were nine children in the classroom, but eight were marked as present on the attendance list. During the visit, the staff corrected the attendance.
OUTDOOR PLAY AREA -Three play kitchens and one play tool bench was not secure.

A review of the program’s medications indicated: -One (REDACTED), for a child enrolled in the program, did not have a prescription label. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed.
A review of four Individual Health Care Plans (IHCP) indicated: -Four IHCP's were incomplete.
