At the time of the visit three staff files were reviewed and the following was found: all three files were missing the EEC orientation checklist form.

The following was observed and accessible to children: TODDLER 1 -An emergency evacuation crib mattress was observed to be covered in plastic covering. TODDLER 3 -There were plastic gloves next to the diaper changing area. -There were two plastic food service gloves on top of the children's cubbies. TODDLER/PRESCHOOL 1 -There were plastic gloves next to the diaper changing area.

Equipment is clean, safe, secured, and poses no hazard.
In the Preschool classroom the following was observed and accessible to children: -The trash in the classroom was not covered with a lid. -The broom and dustpan were laying on the floor. -There was a plastic trash bag on the floor in the bathroom with soiled underwear.

In the Toddler Preschool 1 classroom, two children were observed to be resting on cots that were 2-4 inches apart.
Infant A - During the visit an educator stated that they were "only the assistant teacher" and "did not know" if any children in the classroom had an Individual Health Care Plan.
There were open packages of cereal and rice in a cabinet in the kitchen.
An emergency medication for a child enrolled in the program was not available to them in the Toddler 3 classroom. The name of the child was shared with the Director in the exit interview.
