The following safety hazards were observed and accessible to the children in the indoor licensed space as follows: PLAYROOM/LIVING ROOM: The kitchen play set was not secured to the wall. BATHROOM: Cleaning products, including but not limited to laundry detergents and dish soap, were observed to be accessible to the children under the sink.

Safe Sleep
An (REDACTED) was observed to be sleeping on the couch with a blanket on top of him. Provider reported she had put him down to answer the door as he fell asleep on top of her.

The children's records were observed for the children present, one child was in need of a medical physical and one child was in need of lead testing results.
Provider asked licensor to observe a room to become license. The room observed was the entry way room. The room had an oven which needed to be removed and chipping and peeling paint on the windows.
Provider and the children were utilizing the front room which is not licensed space.
The provider did not have extra clothing for the children present.

Licensed Space
In the unlicensed space where the children were observed, there were several dead flies on the window, chipping and peeling paint and a toy shelf which was not secured causing a tipping hazard. In the living room, an electrical outlet was not properly installed and outside of the wall and several storage shelves were not secured to the wall. In the kitchen, several cabinets and drawers were unlocked allowing access to hazards including matches, a lighter, small screws, a screwdriver, plastic bags, cleaning supplies, trash bags, plastic bags, small propane tanks, scissors and sharp cooking utensils. In the bathroom the cabinet doors were unlocked allowing access to plastic bags, electrical razors and cords which exceed six inches and were loose.
The water temperature was tested, the water temperature was 149 degrees.
The garage in the outdoor space has broken windows and glass, the water hose was unraveled causing a tripping hazard and a section of the fence lining the driveway did not meet the four feet height requirement.

Provider was present with seven children. All children were over the age of 2 years old. The provider's attendance reflects at least two days in which provider had seven children present with one educator.

Provider is licensed for six children and had seven children present. The provider's attendance reflects at least two days in which provider had seven children present.
The first aid kit was missing the children's emergency cards.
All of the children's records were reviewed. there were two children without an enrollment form, one child with an over due physical, four children without a physical, and none of the children had their lead testing. Of the children who had enrollment packets, they all had missing sections of the packet.
There were no progress reports completed for any of the children enrolled.
The provider was observed to be outside with the children without the first aid kit.
The children were observed to enter the program from outdoor play and eat without hand washing. The bathroom was also observed not to have individual or disposable towels for hand drying.
A wading pool was observed to be in the outdoor space filled with water. The water was observed to be dirty and had not been emptied since it had been last used.
In reviewing the attendance records there were observed to be various dates of care missing. Provider was not accurately documenting the arrival/departure time for children who were attending before and after school. The attendance for today did not reflect all of the children which were present.
