During the visit, the licensor observed the following safety hazards accessible to the children in the kitchen area near the door: - A vacuum placed on the floor, posing a tipping hazard, with its cord accessible - A broom with a dustpan - A used mop
During the visit, one playpen, which the educator stated is used for napping a (REDACTED), was observed to have an extra mattress placed on top of the manufactured mattress. The manufactured mattress was not secured to the bottom as required.

Children’s records for the five enrolled children were reviewed, and it was observed that two children require updated medical information, two need annual enrollment updates, one was missing an enrollment packet, one was missing lead screening information, and one had incomplete permission sections in their enrollment packet.
This follow-up visit inspection recorded no violations or advisories.
Safe Sleep
Records were reviewed for the four enrolled children. Two children had incomplete sections in their enrollment forms, including general permissions and emergency contact cards, one of them was missing immunization records and lead information. Two children were missing their enrollment packets and as well as their medical information.
Educator did not have a second surface for diapering materials. Educator stated that they had not been using the changing surface and had instead been changing the children on the rug or the sofa. The changing surface was observed to be stored inside the closet.
Progress reports were reviewed for the four enrolled children. One child was missing a progress report.

First Aid kit is missing Emergency Cards for the four enrolled children.
There was no thermometer inside the refrigerator to check the temperature.
Upon arrival, the licensor observed a (REDACTED) napping on an additional mattress placed inside the playpen. Educator stated that they added the mattress as they felt the original mattress that came with the playpen was too firm and uncomfortable for the child to sleep on.
Although the licensor did not observe educator feeding the (REDACTED), a bottle was seen inside the child's playpen. Educator confirmed that they had provided the bottle to the child while lying in the playpen before falling asleep.
