General: Based on interview and review of documentation, a menu of food items served was not available for review. The childcare provider described that some children would pack their own lunch, and some parents will supply food items for the center pantry and that a menu is not developed for the items prepared at the center.
General: Based on observation, an indicating thermometer was not located within the childcare center refrigerator. The surveyor was unable to determine whether food safe conditions were maintained.

General: Based on review of documentation the following was found: 1) Staff member (DOH: 09/15/23) had a negative tuberculin skin result on file that exceeded two (2) years and therefore was no longer current. The tuberculin skin result was dated 03/20/23. 2) Staff member (DOH: 09/15/23) had a negative tuberculin skin result on file that exceeded two (2) years and therefore was no longer current. The tuberculin skin result was dated 04/06/23.
General: Based on observation, a long phone charging cable was draped over a toy shelf within the back playroom.

A&D ointment was stored on an end table, the cream was accessible to the children in care. 3.) Toothpaste, deodorant, and shaving cream were stored on the sink in the bathroom of the home.
