922 KAR 2:090. Section 9. Records.
General: Based on review of documentation, the following was found: 1. Four (4) children's enrollment files contained a preferred hospital but lacked a contact telephone number. 2. Four (4) children's enrollment files contained a physician's office listed in place of a preferred hospital. It should be noted that preferred physicians were listed for all four (4) children. 3. Two (2) children's enrollment files did not contain a preferred hospital nor a contact telephone number. It should be noted that all children in care are given new enrollment dates yearly upon filling out their applications. Thus, all reviewed files shared a common enrollment date of 08/14/24.
