General: Based on interview, it was determined that an infant child was forced back onto a cot by a staff member during nap time on 11/25/24. The staff member grabbed the child by their ankle and wrist and pulled them forcefully back onto the cot as they tried to crawl off. This was observed to occur several times by an individual viewing the live-stream footage.

General: Based on interview and a review of documentation, on 07/19/24 a two (2)-year-old child was left unsupervised on the toddler playground from approximately 3:23 pm until 4:07 pm. This was observed on video footage provided by the facility. On 07/19/24 three (3) children were taken to the toddler playground by a staff member to combine classes (approximately 2:54 pm). There were two (2) classes on the playground. Based on interview, the staff member that moved the children indicated that she verbally told the teachers on the playground that these children would now be part of their class. The teachers took the children inside at approximately 3:23 pm. One (1) child was left outside unattended on the playground after the other children went inside with the teachers. The video showed the two (2) year-old child playing on outdoor equipment, followed by the same child trying to enter a door to the building. The facility's doors automatically lock when closed, preventing outside entrance. The child was found by another staff member that was leaving work in her car and saw a child on the playground alone. The staff member called a staff member inside the building at approximately 4:07 pm and the child was immediately taken inside. The child was never missed by either staff member that were supposed to be supervising the child. The child did not sustain any injuries. The facility’s outdoor play policy located in the Staff Orientation & Training Checklist Policy stated the following: “Name to Face is required for All transitions outside of the building and throughout the center. Teachers are to be standing and circulating the playground at all times. A 4-corner playground check is also necessary before leaving the playground.” Based on interview and observation of the video, the name-to-face and the 4-corner playground check was not conducted on 07/19/24 when the two (2)-year-old children were taken inside the building from the outdoor play area. The director shall manage the individual job descriptions and assure implementation of child-care policies and procedures.
General: Based on observation, the following was found: Kangaroo Room (Room #2) 1. A mirror was observed on the wall near the door that was broken on the top corner. The mirror was not in good repair. Koalas Room (Room #12) 1. This room contained a yellow cubby with chipped paint/ surfacing at the base of the unit. General: Based on interview and review of documentation, the facility reported that on 9/21/2023 a child's arm was injured when a staff member tried to take a toy away from her and return her to her classroom by her arm. The facility incident report stated that the child was moved into her classroom by her arm. The child sought medical attention and nursemaid's elbow was diagnosed. General: Based on interview and review of documentation, the child-care center failed to maintain adequate supervision. On 7/22/2022 at approximately 4:00 p.m., a three-year-old child was left unattended in classroom #7 for one minute and twenty-eight seconds. A review of video footage found the child was in the restroom as staff and children exited the classroom. Interviews found that one staff person and nine children transitioned from classroom #7 to classroom #8. The second staff person transitioned to classroom #9. Both staff stated that they did not do a 'name to face' count of the children or check the restroom prior to exiting the classroom. Once the child exited the restroom, he stood in the classroom for one minute and twenty-eight seconds before exiting the classroom and walking through the atrium to classroom #9. General: Based on review of documentation, on 03/28/2022, video footage showed a staff person treated a two-year-old child inappropriately in a classroom. The staff person was sitting on the floor with several children gathered around her playing with blocks. At one point a two-year-old child is seen sitting on her knees directly behind the staff person. The staff person reaches around behind her, while still sitting on the floor and grab the child right above the left elbow and drag the child approximately 2 feet so the child is next to the staff person. The child who was still on her knees loses her balance and catches herself by placing her right hand on the floor. The child then stands up and is face to face with the staff person. The staff person is then seen shaking her finger very close to the child's face and appears to say something to the child. The child then walks away but it appears the staff person is still upset because she is then seen forcefully grabbing an object from a child and placing it between her legs. The staff person is also seen grabbing blocks away from children in an aggressive manner. A second staff person stated that she had seen and heard the staff person in question get loud and be somewhat impatient with the children in the classroom. The staff person in question stated that she did feel as if she was a little too aggressive with the children at times but is now more patient with the children. The staff person in charge stated that the staff in question was placed on probation and was given a coaching session. It should be noted that a prior incident happened on 02/14/2022 with the staff person in question regarding tone of voice and aggression towards children in the classroom.
General: Based on interview and review of documentation, a four-year-old child was left outside on the playground for approximately two minutes on 3/18/2022. The child had hidden in a play structure during the transition from the playground to the classroom. The staff person stated that he thought he only had one child left as he went inside classroom #9. It was not until he saw the four-year-old child pass by the window that he realized that she was still outside. He immediately opened the door and let her inside. Based on review of documentation, a staff person was viewed on a two minute video surveillance not disinfecting the diaper changing pad after each diaper change.
General: Based on interview and review of documentation, on 09/09/21 a two-year-old child was left alone on the playground without adult supervision from approximately 10:45am- 11:00am. At approximately 10:45am, per video survellience and interview with staff, there were two toddler classes on the playground. Room 6A had ten (10) children with one staff and Room 6B had nine children with one staff and a speech therapist. As Room 6A's class was coming inside from the playground, an eleventh child was dropped off and was given to Room 6A's staff person which then gave her a total of eleven (11) children. Room 6A staff person told Room 6B's staff person that she was leaving one of her children on the playground with Room 6B so they both would then have ten (10) children. Room 6A then went inside the building. A few minutes later Room 6B came inside. In the video footage there are nine children that came inside with Room 6B and the speech therapist instead of ten (10) children. Per interview, Room 6B staff person asked Room 6A staff person at approximately 11:00am if she had seen the child in question and Room 6A staff person reminded her that she now had ten (10) children instead of nine children and that she did not have the child in question in her classroom. Per vidoe surveillance, after realizing the child was not in the classroom, Room 6B staff person opened the door leading out onto the playground and let the child back into the building. The child did not appear to be in distress during the time he was outside on the playground per video surveillance. According to Accuweather.com the temperature on 09/09/21 was sunny and 77 degrees. Several of the swings on the playground used by the older children contained hooks at the top of the metal chains that were not completely closed. 2. The climbing structures on the playground used by the older children contained some steps that were missing the plastic/rubber coating over the edge of the stairs and rust was exposed in the areas. The handwashing sinks in the following rooms did not meet the minimum water temperature of 90 degrees Fahrenheit: Room #1, Room #3, Room 5A, Room #9, Room #10, and Room #11. The surveyor checked the water temperature at the sinks with a calibrated thermometer.

General: Based on observation, this regulatory requirement was not met. 1. In Classroom #5, the surveyor observed a toilet brush on the restroom floor and a dustpan on the back of a toilet. The placement of the items promote the spread of germs and were accessible to the two-year-old children in care. 2. In Classroom #5, the surveyor observed two (2) stacks of six (6) child-sized chairs positioned next to the children’s cubbies. The children present in the classroom were age two- years-old. The chairs were within an area utilized by the children, and posed a safety risk due to the height. 3. In Classroom #8, the surveyor observed three (3) child-sized chairs on top of a stack of cots. The placement of the chairs on top of the cot promotes the transfer of germs/dirt from the floor to the cot. It should be noted, the staff member removed the chairs during the surveyor’s observation. General: Based on observation, this regulatory requirement was not met. The following was found: 1. Two of the doors on a line of cubbies in the preschool hallway would not close properly due to broken latches. 2. Excessive tape residue was observed on the window ledge in Room #10 (four-to-five-year-old's). 3. The ceiling exhaust fans in the two bathrooms in the school-age classroom would not turn on. 4. The bathroom doors in the school-age classroom contained a build up of dirt, grime, and dried liquid substances.

General: Based on interview, on 09/09/2021 a two-year-old child from Room 6B was left on the playground without adult supervision for approximately fifteen (15) minutes. The staff person in Room 6B did not report the incident. A staff person in Room 6A reported the incident the following day, 09/10/2021 to the staff person in charge. The staff person in Room 6A stated that she had forgotten to mention it on 09/09/2021 because she was very busy in the classroom. General: Based on observation, on 09/09/2021, a two-year-old child was left on the playground from approximately 10:45am-11:00am by himself without adult supervision. The staff person realized after approximately fifteen (15) minutes that the child was still outside and based on video surveillance, the child was let back inside by the staff person. Per video surveillance, the child did not appear to be in distress while on the playground or once he came back inside but it did appear that his entire left pant leg was wet due to dark coloring of his pants.
