General: Based on observation and interview, three (3) folded mats were ripped exposing the padding inside. Interview with the staff in charge, found the mats were not used for children to nap. The mats were used to provide a soft seating option for children reading books.

Review of the schedule found it was time for snack; however, viewing a video was not on the schedule or posted planned program. Interview with the teacher revealed the children were getting a "treat". The video contained community helpers which was reflected as the topic for the week on the posted program plan.

General: Based on interview and review of documentation, the following was found: 1.) Two (2) staff members failed to obtain fifteen (15) hours of training during the previous year and by 06/30/2022. A staff member whose date of hire was identified as 08/13/2018, received eleven and a half (11 1/2) hours and a staff member whose date of hire was identified as 07/17/2013, received fourteen (14) hours of training. 2.) A staff member whose date of hire was identified as 04/20/2021, has yet to complete state approved Orientation training. This staff member has been employed for more than 90 days. 3.) General: Based on observation, this regulatory requirement was not met. The following was found: 1. The larger playground contained several swings with rust on the metal chains. Equipment shall be free of rust. 2. The bathroom in Room 145 contained a sink in which the drain was slow to empty the water. As the surveyor checked the water temperature at the handwashing sink, the sink filled with water and it did not drain properly. General: Based on observation, review of documentation, and a review of the Training Records Information System (TRIS), the following was found: 1. Two staff (hire dates: 1/8/18 and 8/28/19) did not have evidence of completing the required ECE 202 training class (Requirements for Reopening Child Care in Kentucky). The staff person with a 1/8/18 hire date was observed working with a group of children in Room #129 during the inspection. The staff person with a 8/28/19 hire date was identified as a current staff person. 2. A review of staff files during the inspection found that one staff person had a completed criminal records check (CRC) on file dated 5/13/16 and a completed child abuse/neglect check (CAN) on file dated 1/23/18. An administrative staff person stated that the staff person "retired for a couple of years" and then returned to work at the facility on 7/28/20. This staff person is a re-hire; therefore, new background checks are required. The surveyor observed this staff person working alone with a group of children in Room #140 during the inspection.

Based on record review and interview, this regulatory requirement was not met. On 6/29/18 the facility reported that on 6/26/18 at 9:30AM a two (2) year old male child climbed on a wooden shelf in the infant/toddler classroom. The shelf tipped over and landed on the child's ankles. The child initially had red marks across the top of his ankles and later had bruising. General: Based on Review of Documentation, there was one (1) staff person with a hire date of 8/12/14 who had the results of a criminal records check dated 9/17/14. There was no evidence provided for review to verify that the application for the results of the criminal records check was submitted prior to or on 8/12/14 which is the staff persons date of hire. A review of staff files also found that a staff person hired 8/11 did not have the results of a child abuse and neglect check on file at the facility. A review of the staff schedule found that from 5:00-6:00 p.m. the staff person works alone. The staff person in charge confirmed that this staff person is alone with children from 5:00-6:00 p.m.
General: Based on Observation, during a tour of the outdoor play area there was a Litte Tikes slide that had several steps that were cracked.
General: Based on Interview, it it was found that the parents take turns sending in snacks for the class for the week. However, room # 133 does not post a weekly snack menu based on the items the parents send. General: Based on Review of Documentation, there was one (1) staff person hired 8/23/13 who did not have the results of a T.B. skin test read until 8/26/13.
General: Based on Review of Documentation, there were two (2) staff hired 8/15/13 who did not have the application for a child abuse and neglect check submitted until 8/28/13. Both of these staff did not have evidence that the application for the results of a criminal records check had been submitted. There were two (2) additional staff hired 8/20/13. One (1) of these staff did not have the application for the results of a child abuse and neglect check submitted until 8/28/13 and the other staff person did not have an application submitted until 9/6/13. At the time of the visit two (2) of the staff members were left outside alone on the playground with a group of sixteen (16) children without the results of both records checks. These staff were left outside while another staff person brought a child inside who was not feeling well. This staff person then went back out to the playground.
General: Based on Observation, during a tour of the room 140 a cloth bag with an open bottom was observed hanging from a shelf in the bathroom used by the children. The cloth bag contained empty plastic Walmart bags that were accessed through the opening in the bottom. The bags were accessible to the children. In room 141 there were empty plastic Walmart bags in the top drawer of a plastic drawer storage container in the bathroom used by the children. The bags were accessible to the children. In room 133 there were two (2) low hooks on the inside of the bathroom door that contained bags holding empty plastic Walmart bags. The bags were accessible to the children.
General: Based on Review of Documentation, it was found that the file for an employee hired on 8-1-07 contained the results of a TB skin test that was dated 8-20-10. This is not within the two (2) year timeframe required by the regulation. In addition, it was found that the file for an employee hired on 8-1-12 contained the results of a TB skin test that was dated 8-9-12. Therefore, the center's KARES report was not updated to reflect the current employment status of each staff member. Additionally, review of the KARES report determined that thirty-two (32) individuals who are no longer active employees with the childcare program listed as current staff. A review of staff files during the inspection found one staff person that did not have the date of employment listed in their file. General: Based on Interview, it was found that the children bring their own lunch and that some children bring juice boxes, water and sometimes milk. General: Based on Review of Documentation, it was found that a staff person hired 8/11 did not have the results of a child abuse and neglect check on file at the facility. A review of the staff schedule found that from 5:00-6:00 p.m. the staff person works alone. The staff person in charge confirmed that this staff person is alone with childern from 5:00-6:00 p.m.
General: Based on Observation, during a tour of room #133 in an unlocked cabinet over the sink there was a bottle of Fluocinolone Acetonide topical ointment, a bottle of CVS brand children's allergy medicine and an Epi-pen Jr. It was also found that the Epi-pen Jr. was not in the original perscription box but was in a clear plastic tube in a clear plastic bag. In addition in an unlocked first aid kit in the same unlocked cabinet there was a tube of Hydrocortisone cream, Cortizone 10 easy relief applicator and a tube of Benadryl itch stopping cream. The items in the first aid kit were not labeled for an individual person. During a tour of room # 145 there was a tube of Neosporin ointment in an unlocked first aid kit in an unlocked cabinet. General: Based on Observation, a half (1/2) gallon of 2% milk was observed in the refrigerator in room 133.
