During a tour of the facility, the surveyor observed three tubes of diaper cream stored on a shelf with the diaper changing pad, the creams were accessible to the children in care. 2.) An unlocked cabinet underneath the kitchen sink housed cleaning chemicals with warning labels that read, “Keep out of reach of children”. Additionally, the locking mechanism on the knife drawer did not catch when the surveyor opened the drawer, therefore the knives were accessible. General: Based on observation and interview, the facility failed to maintain staff files. One staff person did not have a personnel file on site. The staff person in charge stated that the staff person was a new hire that started on 8/10/20; therefore, she had not had a chance to make a file for this staff person. There was no documentation available for the surveyor to review for this staff person.
General: Based on observation, interview, and a review of the Training Records Information System (TRIS), the following was found: 1. One staff person (hire date: 2/13/15) did not have evidence of completing the required ECE 202 training class (Requirements for Reopening Child Care in Kentucky). The staff person in charge stated that this staff person had worked the previous week (8/3/20 - 8/7/20). 2. The facility failed to submit background checks for one staff person (hire date: 8/10/20). There was no evidence or results on file of a completed child abuse/neglect check (CAN) or a criminal records check. The staff person in charge stated that she had not yet filled out or submitted these forms for processing. The staff person was observed working alone with the infant/toddler group during the visit.

Based on interview it was found that the provider will be providing care for infants, toddlers and preschool children. In order for the provider to be able to care for these age groups there would need to be at least two (2) staff including the provider. It was found that in addition to the provider there is another staff person who will cover the afternoon for one (1) of the age groups. General: Based on Observation, it was found that the staff person caring for six (6) children in the preschool, left her preschool classroom to go upstairs, through the dining room to answer the front door. This staff person was also observed to go upstairs to get milk for the children in care and into another room located off of the kitchen to obtain materials for snack time. General: Based on Observation, it was found that the director did not assure that additional staff were available during cooking and cleaning hours. Observation found that there were only two (2) staff present during the time of the survey. One (1) was observed to be caring for the infants to two (2) year olds and the staff in charge was observed to be caring for the three (3) to five (5) year olds. The staff in charge stated that she cooks the meals for her children while the children are in the classroom. This staff was also observed to leave her classroom of six (6) preschool aged children to go upstairs to get milk and to go into another room located off of the kitchen to obtain items for snack time.
