General: Based on interview and review of documentation, at the time of inspection, there were two (2) staff members (DOH: 11/11/24 and 08/21/25) working alone with the children without either staff member having training or certification in adult CPR and First Aid. The staff were alone with the children for approximately forty-five (45) minutes until a certified (with CPR and First Aid training) staff member arrived at the facility.
General: Based on observation, when the surveyor arrived for inspection, it was observed that all of the children present were on their phones and continued to engage with their phones until they were picked up for the day. One (1) child's time on her phone was observed as thirty-seven (37) minutes of watching YouTube videos without staff screening for the content of the videos. The children were not offered any alternative activities during the inspection.
General: Based on review of documentation, the following was found: 1) Two (2) staff members (DOH: 11/11/24 and 9/04/24) had not completed the six (6) hours of cabinet-approved orientation training which was required within the first ninety (90) days employment. 2) One (1) staff member (DOH: 09/04/24) had not completed the one and one-half (1.5) hours of pediatric abusive head trauma training that was due on or before 09/04/25. 3) Three (3) staff members (DOH: 08/24/23, 09/04/24, 02/27/20) did not have the required (15) fifteen hours of annual training. A review of ECE-TRIS verified all of this information.

Nine out of ten children’s files reviewed did not have authorization for the child to receive emergency medical treatment while in the care of the facility. General: Based on Observation, the surveyor entered the facility at approximately 8:05 a.m. and observed fifteen (15) school age children (11-13 years old) and one (1) staff person. The surveyor asked if she was the staff person in charge. She stated that the staff person in charge had left because she was sick. The staff person later called the staff person in charge when she could not locate files. The staff person in charge returned to the facility at approximately 8:25 a.m. Staff files presented for review found that the staff person who had been left alone when the staff person in charge left the facility was not certified in CPR and First Aid. In short, the staff person was working alone with the school age children for an undetermined amount of time and is not certified in CPR and First Aid.
