General: Based on observation, this regulatory requirement was not met. The surveyor toured the infant classroom at 2:13PM and observed a child standing up in a crib. The only item in the crib with the child was an empty bottle. The surveyor toured the classroom two additional times during the visit and observed the child still standing up in the crib. No activities or toys were available to the child during this time. It is important to note that the staff person working in the classroom was sitting in an adult chair in the room at the time and observed using a breast pump on herself. Premises In Compliance
General: Based on review of documentation and interview, the health, safety and comfort of children in care was not maintained. A witness reported seeing a staff person in the kitchen with several two-year-old children during the morning hours on 08/24/2021. After interviewing the staff person in question, it was determined that she was in the kitchen putting away milk and other breakfast items on the morning of the 08/24/2021. She stated that she had six or seven two-year-old children with her during that time but they were all seated on the floor in the doorway of the kitchen and were never actually in the kitchen. She stated that after putting the breakfast items away she also did some prep work for lunch by placing a pot on the stove. She stated that the stove was never turned on in the presence of the children. She stated that after approximately ten minutes she took the children back to their classroom.
General: Based on interview, a staff person stated that she took her two-year-old class with her when she returned breakfast items back to the kitchen. She stated that she had six or seven two-year-old children with her during that time but they were all seated on the floor in the doorway of the kitchen and were never actually in the kitchen. General: Based on interview, both a five-year-old child and a six-year-old child were permitted to stand on a stool in the kitchen and watch the staff person in charge wash dishes. The staff person in charge confirmed that the children were allowed in the kitchen, but reported they were not allowed to touch the water. Children's Records In Compliance
General: Based on interview, the kitchen door was left open while the staff person in charge prepared meals. The staff person in charge stated that she left the kitchen door and the ‘garage door’ at the counter open so that she could hear the children playing. This was a frequent occurrence during the summer. Interviews found that a five-year-old child helped the staff person in charge throughout the day and often missed activities taking place in the classroom. Occasionally, a seven-year- old child helped the five-year-old child deliver the "food tote" to classrooms. The staff person in charge confirmed that two children were allowed to open the front door of the facility. The staff stated that she would be in the doorway of the gym watching the children answer the door. If the children knew the person at the door, they would allow the person inside the building and if they did not know the person at the door; they would run down the hall to the gym and tell the staff person in charge. General: Based on interview, the surveyor found that the staff person in charge failed to ensure children are adequately supervised. Interviews found that during the summer, approximately ten children, ages five- to eight-years-old, played in the gym while the staff person in charge prepared meals. The staff person in charge stated that she would leave the kitchen door and the ‘garage door’ at the counter opened so that she could hear the children playing. She indicated that since the children were school age, they could be supervised within scope of vision or range of voice. A substitute staff member that was present during the renewal survey conducted on 7/01/2021, did not have a personnel file on site. Based on interview, this staff member was hired on 5/30/2017. The staff in charge indicated that the staff member works at another center that is under the same ownership and she helps at this location when needed. She stated that she was sitting in a chair at the diaper changing cabinet with the doors open getting diaper supplies out. She stated that when she closed the cabinet, the child cried out and she realized the child's finger was smashed in the door. She stated that she did not realize the child was standing up against the right side of the cabinet and had his finger(s) in the doorway. General: Based on observation and interview, the staff person in charge stated that on 01/29/2020, a nine-month-old child got his finger smashed in a diaper storage cabinet in the Wobbler classroom when a staff person shut the cabinet door without knowing the child was in contact with the door. The staff person involved in the incident stated that she was at the diaper storage cabinet retrieving diapers and did not know the child had his finger in the door when she closed the cabinet. The teacher stated that he was the only child in the classroom when the incident occurred and she immediately administered first aid to the child. The child's parents were notified. Based on interview with the parent, the child's fingernail came off and he received stitches internally and externally to the tip of his finger, but the doctor stated that the child's finger should heal correctly over time. General: Based on observation, this regulatory requirement was not met. During a tour of the infant room, three (3) transition bottles were observed inside the refrigerator. The transition bottles were stored in the shelves on the inisde of the door. It was found that the three (3) transition bottles, which contained a white liquid, did not have a cap, lid or other form of cover over the exposed nipple.

General: Based on observation and review of documentation in the Kentucky National Background Check Service (KARES) database, the surveyor found a staff person hired on 4/1/19 had fingerprints submitted on 7/9/19 and an eligible for hire status dated 7/28/19. Per documentation presented for review and a review of the KARES database, the staff person was fingerprinted after being hired. The director stated the staff person transferred from another Creation Kingdom location and her fingerprints had to be submitted a second time. General: Based on observation, this regulatory requirement was not met. The following areas were unclean or in disrepair: 1. Four (4) classrooms contained areas under the hand washing /water fountain sinks that the surfacing was peeling off inside the cabinets. It appeared that there might have been water leaks under these sinks at some point. This was observed in the following classrooms: Room 121 (Yellow room), Room 130 (Green room), Room 127 (Blue room), and Room 125 (Purple Transitional Kindergarten room). 2. Room 127 (Blue room) contained a black stain in the middle of the carpet near a shelf that contained puzzles. 3. The bathrooms in Room 127 (Blue room) and in Room 125 (Purple Transitional Kindergarten room) contained vents in the ceilings with a large accumulation of dust.

This staff member has lived in Georgia within the last five (5) years. The facility conducted a child abuse and neglect check from Georgia, but there was no written evidence that the sexual offender registry check was conducted from Georgia when the staff member was hired. General: Based on observation, interview, and review of documentation, the surveyor found that staff were not following their written planned program (lesson plan) in each classroom. A review of lesson plans found the theme for the week was Boats and Ships. However, there were no thematic materials available to the children in the classroom or available for staff to implement the activities. Some of the activities listed on the lesson plans included: place boats in water table, provide books about boats and ships, put blue dye in the water, and add Noah’s Ark animals, and provide boat file folder games. General: Based on observation, the surveyor found soap scum, grime, and remnants of food in sinks located in Classroom 104, Classroom 108, Classroom 121, and Classroom 125. Additionally, the surveyor observed some milk on the floor in Classroom 125.

General: Based on observation, the surveyor found two preschool sized toilets in the restroom located between Classroom 127 and Classroom 130. The surveyor noted that there were ten school-age children ages five-to ten-years-old present in Classroom 130. The surveyor inquired about the school-agers' restrooms, and the staff person stated that the school-agers use the preschool sized toilets. The surveyor noted that the preschool sized toilets are not a suitable size for the school-age children.
General: Based on observation, the surveyor entered Classroom 130 at approximately 10:00 a.m. and observed a ten-year-old female sitting in a chair that was not suitable for her size. The surveyor noted the child could not place her legs underneath the table. General: Based on interview, a five-year-old child often missed activities taking place in the classroom because she helped the staff person in charge with staff duties. For example, she pushed the lunch cart to each classroom and carried the tote of food into the classrooms; she delivered cleaning supplies, such as the mop and mop bucket, bottles of sanitizing solution and toilet paper. Staff used the walkie-talkie when they needed the supplies, and the five-year-old child would bring them the supplies. The surveyor observed toilet plungers positioned on the floor next to toilets in three (3) bathrooms (Two Year Old Classroom, Preschool Classroom and School Age Classroom). This manner of storage is not sanitary and promotes the spread of germs. 2. The surveyors entered the Infant Classroom at approximately 9:15AM, and observed an 11-month-old child buckled into a Bouncy seat. The child was wiggling around in the seat and attempted to turn over. Putting a child this age in a bouncy seat is counter to recommendations from the manufacturer's guidelines. The guidelines state that the use of Bouncy seats should stop when the child is able to sit up unassisted. It is important to note that the staff person working in the classroom was sitting in an adult chair in the room at the time and observed using a breast pump on herself.
