General: Based on observation and review of documentation, the surveyor entered the playground area of the facility at approximately 9:37 am and observed one staff to be supervising 11 children aged two to six. The staff person in charge was outside of the fenced playground area with two children, while the remaining nine children played inside the fenced playground area. The staff person in charge stated that the second staff person became ill and had to leave, and that she was having someone pick up her own three children in order to correct the ratio issue. This staff person stated that the other staff person had left approximately five minutes before the surveyor arrival. General: Based on observation, it was determined this regulatory requirement was not met. Upon entry to the residence, the surveyor was met by two (2) dogs in the front room. The surveyor entered the kitchen area with the director, which is open to the front room, general classroom area. While the surveyor was speaking with the director, a child stated that one of the dogs had pooped on the floor. Upon inspection, the surveyor did see dog feces on the floor of the general classroom area. The director stated she did not know when the incident could have happened or how long the feces had been on the floor; therefore, the timeframe is unknown. General: Based on interview, Staff #1 has shared personal information about the children in her care and their parents with other parents and providers in her surrounding community. Staff #1 stated that she gives parents or children nicknames as to not divulge confidential information. However, other outside providers are made aware of specific details of situations that happen at the center by Staff #1. This causes them to immediately know details of a parent's current situation at Elna's Angels, which in turn, has caused outside providers to phone Staff #1 letting her know when parents are looking for other child care options. Confidentiality includes sharing personal information about children and/or their parents with other parents, other providers and the public, even when using pseudonyms. General: Based on Observation, in the bathroom used for diaper changing there was a wall with a chunk of the drywall missing. The director stated that her dog had recently chewed that area of the wall.
General: Based on Observation, there were two (2) sinks in the bathroom used for diaper changing. The water temperature at these sinks were tested with a calibrated theremometer and measured approximately 124 degrees Fahrenheit. The director had two (2) thermometers in the bathroom that measured the water temperature to be about 100 degrees Fahrenheit.
General: Based on Observation, there were was a tube of Desitin stored in an unlocked drawer on the diaper changing table. Also, there were medications accessible to the children in the bathroom used for diaper changing. The medications were stored in a drawer equipped with a child safety locked that allowed the drawer to open a few inches. There were several medications that could be removed from the drawer while the child safey lock was still engaged. Outdoor Play Area
General: Based on Review of Documentation, the children's attendance records were kept in a binder. The same binder was used for all the enrolled children and the children's records were separated with tabbed dividers. The children who received services through the Child Care Assistance Program were signed in/signed out on the form required for that program and the child who did not receive services signed in/signed out on a different form. Therefore, it was not kept confidential the children who participated in the Child Care Assistance Program. The director stated that the parents know to only turn their child's tab but there was no way to ensure that is what was actually done by the parents. 130 - Volunteer/Board Member Compliance Not Applicable 922 KAR 2:110. General: Based on Observation, there were items on the vehicle that presented a hazard. Items on the vehicle included a tow hitch, Armor All Cleaner, brake fluid, a screw driver, and other various tools.
General: Based on Observation, the following items were located in the screened porch area, on the top of a table that measured approximatley thirty (30) inches tall: one (1) gallon of Enterprise Semi-Gloss Enamel paint, one (1) gallon of Olympic High Glass Enamel paint, one (1) gallon of Olympic High Glass paint, seven (7) cigarette butts, and three (3) pieces of wooden trim with the sharp end of the nail exposed through the wood. The items located on the thrity (30) inch tall table were accessible to children within the facility. The manufactory label on the paint read, "Keep out of reach of children." Based on observation, twelve (12) pieces of feces were located on the floor of the screened porch. The feces was directly accessible to children in the facility. Also, two (2) propane tanks were loacted on the floor of the screened in porch area. The propane tanks were located by the wooden steps. Based on observation, the following items were located in an unlocked shed on the outside play area: one (1) shovel, one (1) ground tiller, one (1) rake, and seven (7) gallons of paint. Also, an orange extension cord was found drapped across the side of the home. The extension cord was accessible to children. The provider stated that the children enrolled in the facility will sometimes walk through the screened in porch to access the outdoor play area.

General: Based on Observation, a large knife was found in an unlocked drawer in the kitchen island. The knife was found in the second drawer from the floor. Four (4) razors were found in the bathtub area of the toilet room used by the children. The razors were located on a low shelf in the bathtub. Thirteen (13) cleaning chemicals with the manufactory label, “Keep out of reach of children,” were found under the sink in the kitchen area. This shelf was unlocked and accessible to children. Five (5) bottles of soap and shampoo was found in the bathtub area of the toilet room used by the children. These bottles contained the manufactory label, “Keep out of reach to children.” Two (2) pieces of door trim were located beside the entrance to the toilet room door. The trim had long nails exposed and was accessible to children. Several long nails and screw were sticking out of the plywood used to cover the underpinning of the home on the outdoor play area. The sharp edge of the nails was accessible to children. Also, on the outdoor play area, chicken wire had been used to cover the under pinning of the home. The edges of the chicken wire were exposed and sharp. The laundry room area of the home was accessible to children. There was floor cleaner in the room that read, "Keep out of reach of children."

General: Based on Observation, the trim of the flooring between the kitchen and living room was broken and created a tripping hazard. The trim around the toilet room used by the children was broken and had created flaking of white paint and drywall. A drawer in the kitchen was broken and missing a handle. The drawer was located behind the trash can and was accessible to children. The side panel of the kitchen island was coming apart from the base. The trim of the island had broken off, creating sharp wooden edges around the island counter space. The counter space in the kitchen was missing several pieces of trim around the edges.
General: Based on Observation, the trim of the door frame in the toliet room used by the children had white paint flaking from all ends. A knitted rug located by the sink in the kitchen was not secured to the floor, creating a tripping hazard.
