General: Based on observation, the following was found: 1) In the Pre-K classroom, the freezer had a temperature reading of nine (9) degrees Fahrenheit. 2) In the basement Kitchen, the white refrigerator, located next to the oven, had a temperature reading of fifteen (15) degrees Fahrenheit. Therefore, any frozen food was not kept at the required zero degree temperature.
General: Based on observation, the following was found: 1) In the Pre-K Classroom, there were deceased insects in three (3) different light fixtures located in the middle of the classroom on the ceiling. 2) In the Toddler Classroom, there was an accumulation of dust on the vent located near the gray cabinet.
General: Based on observation, in the Three-Year-Old Classroom, the water in the bathroom faucet read eighty (80) degrees on a thermometer. The sink also had low water pressure from the faucet.

General: Based on review of documentation, a staff member (hire date: 8/10/2020) did not have evidence of completing 15 hours of annual early care and education training. General: Based on review of documentation, this regulatory requirement was not met. A review of staff files during the inspection found that three staff members (hire dates: 3/22/21, 4/14/21, and 5/10/21) had a completed criminal records check (CRC) and a completed child abuse/neglect check (CAN) on file; however, the facility has yet to initiate the process for obtaining a background check for these staff members in the Kentucky National Background Check Service (KARES). The staff in charge stated that she misunderstood the email from the Division of Child Care (DCC) about the grace period for completing the KARES check in addition to CAN and CRC. The staff in charge explained that new staff have completed the KARES check, but those three staff members with the CRC and CAN did not, as she thought that was all she needed to do. General: Based on observation, in the Infant room, two children were sleeping in cribs. These two cribs were only seven inches apart from one another. General: Based on interview, the staff in the Two-to-Three-Year-Old Room indicated that children are rewarded throughout the day for good behavior and then they receive a treat at the end of the day. The special treat consists of two Tootsie Rolls. The staff person stated these are given to the child when they go home. The staff person indicated further that on Friday of each week, those children who have their card filled up receive an additional special treat, such as brownies, as part of their afternoon snack. It was also stated that those children who do not fill their card up by the end of the week are not given a brownie or special treat. Food is not permitted to be used as a reward or for disciplinary purposes.
General: Based on observation of naptime in the Two to Three-year-old room, at the time of the investigation, staff were observed telling a child they would remove a sticker from his chart if he did not lay down on his mat and go to sleep. The staff then removed the sticker because he would not lay quietly on his mat and go to sleep. This regulation prohibits discipline associated with rest.
General: Based on observation and interview during naptime in the Two-to-Three-Year-Old room, a child was not permitted to play quietly when he was not sleeping. Staff were observed telling the child they would remove a sticker he had earned earlier in the day from his chart, if he did not lay down on his mat and go to sleep. Staff interviews found that between 11:30 a.m. and 12:00 p.m. they begin preparation for nap time and by 12:00 p.m. all of the children are on their mats. Naptime is from 12:00 p.m. to 2:00 p.m. Staff were also observed telling the child he would not receive a special treat at the end of the day. At 12:21 p.m., the child was still awake. Staff continued to tell the child he needed to lay still and go to sleep. The child was never offered an alternate quiet activity. General: Based on observation, the following was found: 1) A staff member's purse was sitting on top of a low shelf in the two-year-old classroom. The purse was accessible to the children. 2). The door leading into the break room was open and a spray bottle labeled bleach solution and a container of disinfecting wipes were both found on a table. Also in the break room, there was a gray metal cabinet that was unlocked and inside were containers of disinfecting wipes. These items were all accessible to the children.

At the time of the visit, it was found that four (4) staff members were supervising a group of twenty-five (25) children on the playground (age range: 2 - 3 years). Staff stated that three (3) different classrooms were present. The surveyor observed these three (3) groups together from approximatley 10:15AM - 10:47PM. The youngest child in the group was two (2) year old, making the maximum group size twenty (20). The facility exceeded the group size by five (5) children. Based on interview, a four (4) year old child fell off a slide while playing on the playground and began crying holding her left arm. The staff person did not take the child's coat off to see if her arm was okay and did not call the parents. It was later determined that the child's arm was broken. According to the incident report she fell off the slide around 5:25pm and her father picked her up at 5:30pm. However, interview with parent stated that when child was picked up at 5:30pm he was told that the child fell off the slide around 5:00pm but the teacher did not think anything was wrong with the child so no one was called at that time. General: Based on Observation, in the 3 and 4 year olf classroom, a large amount of debris and dust had collected on the ceiling vent. A floor vent in the 4 and 5 year old classroom was bent and several toys were found within the vent.
General: Based on Review of Documentation, it was found that the file for an employee hired on 3-15-05 contained the results of a TB skin test that was read on 5-2-11. This date is not within the two (2) year timeframe required by the regulation.
General: Based on Observation, it was found that a pair of sharp scissors was on the vanity of the sink in the toddler classroom. Also in the toddler classroom, it was found that two (2) purses belonging to the staff were on the low cubbies. These areas are accessible to the children.

General: Based on Observation, a high chair was located in the downstairs hallway. The high chair had no security straps to prevent a child from falling out of the chair. In the toddler classroom, a high chair had no security straps to prevent a child from falling out of the chair. In the transition room, four high chairs did not have security straps to prevent a child from falling out of a chair. In the toddler classroom, the rocking chair cushion had a rip in the material, exposing the stuffing inside the cushion. The rocking chair did not have a support base under the cushion, which deamed it unsafe to sit in. in the transition classroom, a large tear in the rocking chair cushion was found. In the 4 and 5 year old classroom, a Thomas the Train soft chair had a dark brown stain on the front. A red shelf in the 2-3 year old room had paint chips flaking from several sides. A white shelf in the 3-4 classroom science area had paint chips flaking from several sides.

The cot was covered with a sheet and spider man blanket, which was contaiminated by both children. An electrical outlet was uncovered in the infant classroom. In the transition classroom, the protective plate on an electrical outlet was broken, exposing sharp edges.

General: Based on Observation, it was found that the refrigerator in the four (4) and five (5) year old classroom contained spills and debris. There was also a buildup of grime around the device used to secure the refrigerator. In the toddler classroom, it was found that the staff were washing sippy cups, bowls and utensils in the small handwashing sink and then reusing them later in the day.
General: Based on Observation, a covered bowl of applesauce was found on the counter in the transition room. The teacher reported that the applesauce was used for breakfast, but was not discraded or refrigerated afterwards. A Yoplait yogurt was found in the infant classroom refrigerator. The yogurt was opened and contained a spoon inside.
General: Based on Interview, it was found that a gallon of 1% milk was in the refrigerator in the toddler classroom. The teachers stated that the milk was served to the children in the classroom. All of the children in this classroom were one (1) year old.
General: Based on Observation, children were served a lunch consisting of crackers, cheese slices, mixed fruit, and chicken noodle soup. No vegetable or second fruit was offered to children during the lunch service. Children's Records
