General: Based on observation, it was found that the weed guard was exposed in numerous areas of the playground. This created a tripped hazard for the children in care. General: Based on observation, this regulatory requirement was not met. During a tour of the facility, the following was found: 1) In the room referred to as the Free Room, an adult purse was observed sitting on the floor by the office. A toddler was observed walking in and out of this space throughout the duration of the survey. The purse was accessible to the children. 2) An adult purse was also observed on the second shelf of a low shelving unit in the room where the cribs are located. The purse was within reach of those children who are walking.
General: Based on observation and interview, this regulatory requirement was not met. During an inspection of the bathroom used by the children and staff the following was found: 1) The water temperature at the sink was 140 degrees Fahrenheit. This exceeds the maximum allowable water temperature of 120 degrees Fahrenheit. 2) The changing table used for diaper changing is located in the room with the cribs. The changing table is positioned near the doorway leading into the kitchen. An interview with the staff person in charge found that after a child's diaper is changed they must go through the kitchen and open the door that leads into the bathroom to wash their hands and the child's hands. During a tour of the facility the following was found: 1) An inspection of the bathroom found a tube of Neosporin antibiotic ointment on the counter. The ointment was not in a locked container or area and was not labeled for an indivdual child or adult. 2) An inspection of the diaper changing table found a tube of eczema lotion in an open compartment on top of the table. The label on the tube of lotion contained drug facts and a warning to keep out of the reach of children. The tube of lotion was not in a locked container or area and was not labeled for an invidual child or adult. 3) During an inspection of the kitchen, a bottle of Infant Tylenol drops was observed sitting on the counter by the sink. The bottle of Tylenol had been opened and was not labeled for an inidividual child. The Tylenol was not in a locked container or area. In an interview with the staff person present it was stated that the child has issues with reflux and the parent asked that the child be propped up when sleeping. At the time of the inspection, the following was found during a tour of the kitchen: 1) The floor in the kitchen had a sitcky substance in several areas. 2) A can of Hot Shot Roach Killer was observed on the counter by the refrigerator. A head of lettuce, a small blue bowl conatining part of an onion, a can of fruit, a jar of pickles, a container of Country Crock butter with the lid removed and various other food items were observed stored on the counter in the same area. 3) A bottle of Pine Sol and mop was observed on the counter near a red Kurig coffee makre. A package of opened taco shells was observed in the same area on the counter. This is not an appropriate method of storage for food and hazardous chemicals and cleaners.

General: Based on Interview, it was found that the two-year-old children were being taken into the Infant classroom for diaper changing on a daily basis. There was no other changing area provided for the older children in the facility.
General: Based on Observation, the water temperature at the sink in the bathroom registered one hundred, twenty-six (126) degrees Fahrenheit when measured with a calibrated thermometer. The children use this sink for hand washing.
General: Based on Observation, 1. There were five (5) bottles of pre-measured water in bins in the Infant classroom that were not labeled with a child's name. 2. There were three (3) bottles sitting out that were not in use and not covered. 3. There were two (2) bottles of formula sitting out on a shelf. The bottles were not in use and were not being refrigerated.
General: Based on Observation, there was no information from the Kentucky Consumer Product Safety Program as specified in KRS 199.897 posted in the facility.
