During the survey of the Toddlers #2 room, it was found that a patch of wall along the right side of the room had blue, red, and black marks on it that went in circular and random patterns. This exceeds the ninety-day time frame. There were two (2) staff (hire dates: 8/10/15, 8/12/16) who did not evidence of completing fifteen (15) hours of training for the 2017-2018 training year. There were two (2) staff with the hire date of 8/1/17 who did not have evidence of completing fifteen (15) hours of training for the 2017-2018 training year. In addition, a staff person hired 8/10/15 had completed the required pediatric abusive head trauma training on 6/4/12. This staff person has not renewed this training as of the date of this survey. General: Based on Observation, at the time of the visit there were two (2) rabbits, fish, a turtle and a Gecko at the facility. A review of children's files found that the turtle and Gecko had not been listed on the permission form for animals the child would be in contact with while at the facility.
General: Based on Review of Documentation with the staff person in charge it was indicated that documentation verifying the monthly fire drills for 2015 until the present date had been discarded.
General: Based on Review of Documentation, there were two (2) staff with hire dates of 8/10/15 who were eligible for an annual performance evaluation. However, there was no evidence on file to verify that an evaluation had been conducted. During an interview with the staff person in charge it was indicated that evaluations had not been conducted.
General: Based on Review of Documentation, there was one (1) staff person with a hire date of 8/12/16 who lived in Illinois within the last five (5) years. There was an additional staff person with a hire date of 8/16/16 who lived in Wisconsin within the last five (5) years. The results of a criminal records check from these states was not on file nor was there evidence that an application for the results of a criminal records check from these states had been submitted. During an interview with the staff person in charge it was found that these staff work together with children in the afternoon. It was also found that these staff are the only staff who are present.
General: Based on Review of Documentation and interview, the surveyor reviewed fire drill documentation for the month of October and found that the names of the children who particpated in the drill were not documented. The director stated she did not write down the names of the children who particpated in the drill.
General: Based on Review of Documentation, one (1) child's file contained an immunization certificate that was not dated. The surveyor could not determine if the immunization certificate was current. (2) Two (2) children's files contained immunization certificates that were no longer current as of 9/22/15 and 10/11/15. (3) One (1) child's file did not contain immunization documentation. This child has been enrolled in the facility since 8/12/15.

During the survey of the children's bathroom in the Toddlers #2 room, it was found that the toilet did not flush properly when tested by the surveyor. The water did not go down the drain and began to rise upon being flushed, indicating a malfunction or an issue with clogging. General: Based on observation in the Dandelion Room an electrical cord connected to a light table and an electrical cord connected to a room diffuser were observed plugged into an outlet located under the window. The diffuser was sitting in the window. The cords hung loosely from the wall. Part of the cord from the light table was observed lying on the ground. A child- size stool straddled the cord. Children were observed sitting on the stool and standing around the light table. The cords were accessible to the children and caused a tripping hazard due to their location. General: Based on observation there was a sensory table located in the outdoor garden area used by the children. The basin of the sensory table was cracked in one (1) corner. There were other areas that appeared to have been repaired with purple and white Duct tape. A child sized watering can and a puddle of water with what appeared to be a small amount of sand were also observed in the basin. In the large outdoor area used for gross motor play there was a large wooden spool shaped object sitting on it's end forming a table with child sized chair sitting around the outside edge. The wood surface of the spool was rotted and splintered. General: Based on Observation, at the time of the survey an opening (doorway) into another classroom was observed. An interview with the staff person in charge found that over the summer the doorway had been made from an existing wall. The doorway leads to another classroom. General: Based on review of documentation, there were three (3) staff (hire dates: 9/28/18, 9/25/18 and 9/4/18) who did not have a high school diploma, GED or other qualifying documentation on file at the time of the survey.
General: Based on observation in the outdoor play area there were two (2) large and tall tree stumps and three (3) large tree trunks lying on their side. An interview with the staff person in charge found that the stumps and the logs are used for the children to sit and climb on. The mulch around these areas was sparse. Dirt and grass was visible in many areas around the logs and stumps. In the same area there was a tree with a knotted rope attached to the bottom branch. A child sized chair was observed sitting beside the rope under the tree The staff person in charge stated the children are allowed to climb the rope and that the chair is used for them to stand on to help them get on the rope. General: Based on observation an uncovered stick of butter was observed sitting on a plate in the refrigerator located in the kitchen. In the refrigerator located in the Lavender Room a bowl of sliced red peppers and a bowl of sliced avocados was observed sitting in uncovered bowls on the bottom shelf. During a tour of the preschool room small popcorn kernels were observed as part of one of the activities available on a shelf. The children in the preschool room were three (3) to five-years-old. The same type of rocks with sharp edges were also observed at the base of the climbing wall located on the other side of the dirt mound.
