General: Based on interview and review of documentation, a child named in an allegation (DOE: 11/20/25) did not have enrollment information available for review at the time of survey. Therefore, the child had attended the child care center without signed enrollment information containing his information, parent or guardian information, preferred physician, preferred hospital, medical history, authorized persons to pick-up the child, nor authorization to seek emergency medical care if needed. The surveyor requested to see the documentation to gather information for the investigation and was told that the parent had not completed and returned it as of the date of survey, 12/01/25. The staff in charge verbally identified the child's first day of attendance as 11/20/25, further stating he attended on 11/20/25 and 11/24/25, and no other dates. It should be noted that an immunization record was available for review as well as one (1) parents name and contact telephone number.
