The surveyor also noted that a staff person left the two-year-old classroom at 11:24am-11:27am and went outside; then left again at 11:28am to go to the bathroom. General: Based on Observation, upon the surveyor's arrival to the facility on 6/13/14, a staff member came to the front door and unlocked it and the surveyor and the staff member walked through the building and then the staff exited the building onto the playground. The surveyor stopped in the preschool room and observed a child in the room alone. The staff member that allowed the surveyor into the building indicated that he was supervising a group of eleven (11) preschool children with another staff outside on the playground. The surveyor observed another individual and child on their way to the back of the preschool room and this individual was asked by the surveyor if the child found in the classroom alone was assigned to her care. The individual denied this and stated that she was dropping off her child. The individual and the surveyor took the child outside to a staff member. The child was indentified as being a one-year-old. On 6/13/14 the staff member who was initially caring for the one-year child indicated that this child had began pulling at her ear and acting as if she did not feel well. The staff member then took the one-year-old to the director who said she would supervise the child. This was confirmed in interview by a second staff member on 6/16/14. The staff member responsible for the one (1) year-old class indicated that she then took her group of toddlers outside while the director still had that particular child in her care. The staff member responsible for the one-year-olds said that some parents came and she observed the director talking with them. The staff member supervising the one-year-old class indicated that her class had been on the playground for approximately thirty (30) minutes prior to the surveyor's arrival. Another staff stated her class of children had been on the playground approximately ten (10) to fifteen (15) minutes prior to the surveyor's arrival. At the time the surveyor arrived the director, who was responsible for the supervision of the one-year-old child was outside on the playground. It is unknown how long the one-year-old was unsupervised inside the building. The staff in charge indicated that she had not conducted criminal records checks on the volunteers. In addition, a substittue staff member that was re-hired on 10/15/12 did not have written evidence of a new criminal records check being conducted.
