General: Based on review of documentation, this regulation was not met. There was documentation of earthquake, tornado, and lockdown drills on file at the facility; however, there was only a time documented under the earthquake drill and then an arrow drawn across without any documented times for tornado or lockdown for the second and fourth quarters. Each drill conducted requires different actions; therefore, must have different times documented.
General: Based on review of documentation, a child (DOE: 07/02/24) did not have an up-to-date record of immunizations on file at the time of inspection. The record on file expired on 01/20/25.

General: Based on Review of Documentation, it was found that two (2) employees did not have the required hours of training for the last 12 month cycle. One employee with a hire date of 4-1-1997 had nine (9) hours of training in the last completed year. The second employee with a hire date of 9-16-16 had ten (10) hours of training in the last completed year. This was verified through ECE TRIS. General: Based on Review of Documentation, there was a staff person with a hire date of 2/23/15 that had a TB skin test result that was no longer valid in February 2017. The staff person in charge stated that she did not realize it had expired. General: Based on Review of Documentation, staff person hired 1/13/16 did not have the results of or evidence of the application for a criminal background check on file in the facility. In an interview with the director it was stated that the facility had gotten a background check from the staff person's previous state of residence but forgot to get one from Kentucky. The director stated that the staff person is never alone with children.
General: Based on Observation, the outdoor sandbox did not contain a lid or cover.
General: Based on Review of Documentation, the facility had documentation of quarterly earthquake and tornado drills that included the date and time of the drills; however, the names of the children who participated were not documented.

General: Based on Review of Documentation, it was found that one (1) staff member had a TB skin test on file that was no longer current. The TB skin test was dated 10/8/10.
