General: Based on observation during the inspection, a child was walking alone in the hallway coming from the courtyard area and going to the front office for dismissal. The surveyor asked the person in charge about their procedure for dismissal and she stated that "the children navigate the school really well" and can walk alone to the office at pick up time. General: Based on Observation, the after-school program uses the library for the children and it contained an aquarium with fish. The facility did not have written parental consent for the children to be in the presence of the fish.

General: Based on Review of Documentation, there were three (3) staff with a hire date of 8/11/15 who had the results of a crminal records check dated after the staff person's date of hire. Two (2) of those staff did not have documentation to verify that the application for the results of the criminal records check had been submitted prior to or on the date of hire. The third staff person had a check signed and dated from the program to be submittied for payment with the application that was dated the day after the staff person's date of hire. In addition there was one (1) staff person hired 8/11/15 whose application for the results of a child abuse and neglect check was not submitted until 12/8/15. This was verified through a note that had been attached to the results page.

General: Based on Review of Documentation, there were five (5) staff whose T.B. tests were completed after their hire dates. The hire date and the date the T.B. skin test was completed are as follows: hire date 1/5/16 had a T.B. test completed on 1/6/16, hire date 3/7/16 had a T.B. test completed on 3/9/16, hire date 11/9/15 had a T.B. test completed on 11/12/15, hire date 1/6/16 had a T.B. skin test copmpleted 2/26/16 and hire date 8/11/15 had a T.B. test completed on 3/21/16, This does not meet the requirement that the results of a current T.B skin test or a statement from a health professional stating the individual is free from the symptoms of T.B. be completed prior to employment.
