The determination status was “In Process”. The staff member was present during the inspection but did leave the child care center after the staff-in- charge was notified by the surveyor that KARES has to be completed, and employee must be determined eligible before starting to work with children. The staff-in-charge stated that this employee had not worked alone with children.

General: Based on interview and review of documentation, this regulatory requirement was not met. During the investigation it was found that staff were not aware that a 13-year-old child with limited English language skills walked out of the building, crossed the street, and went to the gas station. A staff member found the child approximately 10 (ten) minutes later, when she spotted him in the middle of the street with ongoing traffic, coming towards the facility. General: Based on observation during a tour of the Science room, a gallon of bleach was observed in a cabinet under the sink. The label on the bottle of bleach has a warning to keep out of the reach of children. There was no lock on the cabinet, which makes the bleach accessible to the children. General: Based on review of documentation, it was determined this regulatory requirement was not met. An employee with a hire date of 06/01/2018, had not completed fifteen (15) hours of training for the current year as required by regulation. The employee did not have record of completing any training since her hire date. The employee verbally confirmed that she had not completed any training hours during her time at the facility. At the time of the survey it was found that Transportation Cabinet vehicle documentation could not be provided for one (1) of the facility's vehicles used to transport children (2008 Chevrolet mini bus). The file for a staff person hired on 7/10/17 did not contain child abuse/neglect (CAN) results. The file contained a copy of the CAN check that had been submitted for processing; however, the results had not been obtained. This staff person was observed working alone with children in the gym when the surveyor arrived at the facility (3:05PM). The staff person in charge explained the staff schedule by stating that this staff person works with children in the gym alone from approximately 2:50PM - 3:10PM each day and then a second staff person joins him at 3:10PM. Written documentation received in the DRCC office from the staff person in charge at the facility stated that a child lost his balance and fell at the facility on 6/20/17 (Tuesday) and injured his leg. The written documentation further indicated that the child's mother picked the child up after the incident and told the staff person in charge that she was taking the child to be examined by a physician. General: Based on review of documentation, the surveyor found that one (1) staff person (hire date: 12/4/15) did not have the results of a criminal records check nor was there any documentation presented for review in order to verify that the criminal records check had been requested. The Director stated that the staff person's criminal records check was on file at the facility's central office located in Cincinnati. It is important to note that this staff person was not working alone with the children at the time of the inspection.

The posted plan indicated the times that the children would be in each area such as the art room, the game room, and the gym, but the specific types of activities that the children would be doing in each area was not posted.

General: Based on a review of documentation, there were four staff who did not have a copy of a high school diploma, GED or other qualifying documentation on file. The hire dates of these staff are as follows: Two staff were hired 7/2/2020, one was hired on 7/6/2020 and one was hired on 6/30/2020.
General: Based on a review of documentation, it was found that only one staff person is currently certified in CPR and First Aid. An interview with the certified staff person found she works from 7:30 a.m. until 4:00 p.m. An interview with the staff person in charge found that the facility is currently open from 8:00 a.m. until 5:00 p.m. The regulation requires there to be at least one certified person on duty and present with the children during the hours of operation. Therefore, from 4:00 p.m. to 5:00 p.m., a person who is currently certified in First Aid and CPR is not on duty and present with the children as required. A phone interview with the staff person in charge on 7/27/2020, found there are individuals located in other parts of the building who are currently certified in CPR and First Aid. A review of the transportation documentation found that the times that children got on and off the Silver #4 van during the week of April 30 - May 4, 2018 had not been documented. The staff person in charge acknowledged the finding and stated that this vehicle is used to pick up approximatley seven (7) children from a local elementary school daily.
