The door leading into the Preschool room had extreme wear and was in need of repainting. It should also be noted that the door was missing a door knob.

General: Based on observation, this regulatory requirement was not met. The following was found: 1. A rusted floor vent was observed in the Early Preschool Room. The vent contained areas of chipped white paint and rust on the surface. 2. The hall bathroom (used by the staff), across from the kitchen, contained a round electrical box with exposed wiring inside it. The box was located on the wall near the ceiling.

General: Based on observation, the bsthroom inside the Pre-K room contained a blue step stool that was dirty on the surface. The Early Preschool Room contained a pink owl rug in front of the home living center that was stained.
General: Based on observation, the following items were unclean or in disrepair: 1. The bathroom door in the Early Preschool Room contained stains and spills on the surface. 2. The Toddler Room contained a wooden shelf (in the center of the room) with tape residue on the top. 3. A vent in the floor of the kitchen was rusty on the surface. 4. The vent in the ceiling of the hallway next to the kitchen contained an excessive amount of dust. 5. The exit door in the Pre-K Room contained brown stains on the bottom that appeared to be water marks. A food service permit is typically required to perform these tasks. No documentation was available to indicate this permit was not required.
