General: Based on observation, the child care failed to maintain state regulations. During the survey, the following was found: 1). An unfurled water hose laying on the pathway leading to the outdoor play area. This presented a tripping hazard for children in care. 2.) A long wooden board was found propped up beside a park bench on the outdoor play area. 3.) Weeds and overgrowth were found in between the outdoor play apparatuses.
