General: Based on observation, the following was found: Infant Classroom 1. The activity and feeding table seats were not equipped with safety belts. Two's and Three's Classroom 1. Food debris was found in the handwashing sink used by children in care. Preschool Classroom 1. The steps for the handwashing sink had an accumulation of grime.
General: Based on observation, the following was found: Toddler Classroom 1. A child-sized table had a piece of the plastic border missing. 2. The puzzles cabinet had tape residue that had begun to collect debris. 3. The soft climbing apparatus had a tear in the seam. Two's and Three's Classroom 1. The two (2) books available to children in care were damaged. 2. The phones cabinet had tape residue that had begun to collect debris. Preschool Classroom 1. The dramatic play kitchen had tape residue in several areas that had begun to collect debris. 2. The blocks cabinet had a large chip out of the wood on the bottom shelf. 3. General: Based on review of documentation, the following was found: 1. A staff member (DOH: 02/14/25) completed thirteen (13) hours of the required fifteen (15) hours of annual training for the year dated 02/14/25 to 02/14/26. 2. A staff member (DOH: 03/03/25) completed nine and one half (9.5) hours of the required fifteen (15) hours of annual training for the year dated 03/03/25 to 03/03/26. 3. A staff member (DOH: 06/11/25) completed their cabinet-approved orientation training on 11/21/25, after their first three (3) months of hire had lapsed. A review of ECE-TRIS confirmed these findings.
General: Based on observation and interview, the following was found: 1) From 8:45 a.m. until approximately 9:25 a.m., the Pre-K Classroom was observed to be out of ratio. There were sixteen (16) children, aged three to four (3-4) years old with one (1) staff member. The staff member was asked how long she had been out of ratio before the surveyor arrived at 8:45 a.m. The staff member stated "ten (10) minutes." 2) At 8:55 a.m., the Two-Year-Old Classroom was observed to be out of ratio. There were eleven (11) children, aged two (2) years old with one (1) staff member. At approximately 9:15 a.m., a second staff member arrived for that classroom. General Administration Discussed 225 - Licensee Responsibility Discussed 922 KAR 2:090. Section 8. General. (1) A licensee shall: (a) Be responsible for the operation of the child-care center pursuant to this administrative regulation, 922 KAR 2:120, and 922 KAR 2:280; and (b) Protect and ensure the health, safety, and comfort of each child.
General: Based on interview and review of documentation, a staff member placed a five-year-old child into the hallway where he was unsupervised for approximately thirty-five (35) seconds. Staffing Requirements In Compliance
General: Based on interview and review of documentation, a staff member lifted a five-year-old child from a sitting to a standing position by his wrist. When the child dropped to his knees, the staff member pulled him up by his wrist/wrists six more times as she walked him to the classroom door. The child received red marks on his wrist as a result of this incident. Programming Not In Compliance
General: Based on observation and interview, a video that was not part of the planned program was played in the Preschool Classroom for approximately thirty (30) minutes to one (1) hour on 02/27/26. General: Based on observation, the following was found: Two-Year-old restroom 1) A dusty vent was observed in the ceiling over the toilet. Preschool restroom 1) A dusty vent was observed in the ceiling over the toilet.
General: Based on review of documentation, a staff member (DOH: 04/04/25) did not have proof of education on file.
General: Based on review of documentation, the personnel record for a staff member (DOH: 02/14/25) contained the most recent TB skin test dated 03/30/23, which was no longer current.
General: Based on observation, the following was found: Two-Year-old Room 1) A cracked plastic basket was observed on a shelf with play food inside the basket. Preschool Room 1) Tape residue was found on the wooden dramatic play stove and refrigerator set. Toddler Room 1) Tape residue was located on a wooden shelf that was used to store blocks.
General: Based on review of documentation, the following was found: 1) A child (DOE: 05/15/24) did not have a physician’s phone number on file. The area on the enrollment form was left blank. 2) Of the ten (10) children’s files reviewed, none of the files contained a phone number for a preferred hospital.

General: Based on observation, the toilet seat in the restroom of the Preschool Classroom was damaged. Hygienic Practices Not In Compliance
General: Based on review of documentation, a written plan detailing the reunification of infants and toddlers, children with chronic medical conditions, and/or children with disabilities with their guardian in the event of an emergency was not available for review at the time of survey. General: Based on observation, the following was found: Two's and Three's Classroom 1. There were several areas of chipped paint on the birthday wall. Preschool Classroom 1. The wall behind the restroom door had damage to the paint and drywall. 2. One (1) stained ceiling tile was observed above the dramatic play center. 3. The wall near the door exiting to the outside of the building had damage to the paint and drywall. 4. The wall by the wooden blocks cabinet had damage to the paint and drywall, exposing a screw head.
General: Based on interview and review of documentation, the dates of hire for three (3) staff members (DOH: 02/19/26, 03/02/26, and 03/27/26) were not recorded in their personnel files. The staff in charge provided the dates of hire verbally to the surveyor.
General: Based on observation, the door to the restroom in the Preschool Classroom had significant paint peeled away beginning below the handle all the way to the base of the door.
General: Based on observation, three (3) blue storage baskets were found on the diaper changing surface in the Two's and Three's Classroom.
General: Based on observation, a section of weed barrier was protruding through the mulch in front of the red slide on the playground. This poses a potential safety hazard.
General: Based on observation and interview, the Two's and Three's Classroom had an insufficient supply of toys and books available for children in care. The toy shelves in the classroom were bare and the book shelf was empty except for two (2) damaged books. The staff member present stated the books had fallen behind the bookshelf. It should be noted approximately ten (10) to fifteen (15) small animal and dinosaur figurines and toy cars were scattered across the floor of the classroom.
General: Based on review of documentation, the following was found: 1. Of the ten (10) children's files reviewed, nine (9) did not contain a contact telephone number for their respective preferred hospitals. 2. One (1) child's enrollment file (DOE: 12/11/25) did not contain a preferred hospital in case of an emergency. The space available had been filled in with "any" as opposed to a specific area hospital.
General: Based on review of documentation, record of a quarterly shelter-in-place drill having been conducted for the third and fourth quarters of 2025 was not available for review at the time of survey.
General: Based on observation, the lesson plans posted in the Two's and Three's Classroom and the Toddler Classroom were not dated and therefore could not be determined to be current.
General: Based on observation, a diagram was posted showing evacuation routes; however, written instructions for various emergency situations were not posted with it.
General: Based on observation, allergy care plans (regarding food items) for various children in care were not posted where food may be served. The facility had the children's names and the allergy posted, but no care plan was posted.
