General: Based on observation and interview, an allergy care plan was not posted for a child with a peanut allergy in Classroom 200. Interview with the staff in charge and teacher found information regarding the allergy had been obtained, and kept in Brightwheel; however, a care plan had not been posted in the classroom.
General: Based on observation, on the day of inspection, Rooms 46 and 154 had children's coats and jackets hanging outside of the classroom without a barrier to prevent touching one another.

General: Based on observation, Room 153 had an area of unpainted wall with a small area of torn drywall on the day of inspection.
General: Based on review of documentation, the child care center failed to maintain state regulations. At the time of survey, one (1) staff member (DOH: 10/28/24) had not yet taken the six (6) hour orientation training, which is to be completed within ninety (90) days of employment. A review of ECE-TRIS confirmed this finding.
General: Based on observation, the following was found: Room 155 1) A light was not in working order the day of the survey. Room 156 1) A light was not in working order the day of the survey. Room 153 1) A light was not in working order the day of the survey. Room 144 1) A light was not in working order the day of the survey.
General: Based on observation, Room 200 had two (2) stained ceiling tiles on the day of inspection.
General: Based on observation, the refrigerator in Room 142 used for storage of children's food was reading forty-nine (49) degrees Fahrenheit on the thermometer.
General: Based on review of documentation, the child care center failed to maintain state regulations. One (1) child's file (DOE: 08/11/24) did not contain a specific preferred hospital in the event that emergency treatment was necessary.
