922 KAR 2:090. Section 8. General.
General: Based on review of documentation, on 03/17/25 a four-year-old child jumped from a foam flower pad while in the indoor gross motor room, landed directly onto his face injuring his nose. At the beginning of the video footage, the child is seen jumping from the foam flower pad into the outstretched arms of a staff person who is standing within a couple of feet of the child. The video footage then showed the staff person holding onto the child's hands as he jumps off the foam flower pad and gently lands on the ground. The video footage then showed the staff person move several feet away from the foam flower pad while the child is standing on the pad looking at the staff person. The staff person initially has her arms outstretched so the child jumps and as the child jumps, the staff person steps backwards and lets her arms fall to her side and the child falls face first onto the floor.
