General: Based on observation, the bathroom in the Walkers Classroom did not have a supply of toilet paper and the dispenser was empty.
General: Based on observation, the following was found: Discovery Classroom 1. The shelf in the block center had peeled paint on the outer surface, accessible to children. Little Learners Classroom 1. A racecar pillow in the dramatic play center was worn with holes exposing the interior foam. 865 - Indoor/Outdoor Equipment Discussed 922 KAR 2:120. Section 13. Toys and Furnishings. (2) Indoor and outdoor equipment shall: (a) Be clean, safe, and in good repair; (b) Meet the physical, developmental needs, and interests of children of different age groups; (c) Be free from sharp points or corners, splinters, protruding nails or bolts, loose or rusty parts, hazardous small parts, lead-based paint, poisonous material, and flaking or chalking paint; and (d) Be designed to guard against entrapment or situations that may cause strangulation.

General: Based on interview and review of documentation, a staff member failed to change a two (2) year old child's soiled and wet diaper for approximately six (6) hours, resulting in prolonged exposure to unsanitary conditions.
General: Based on interview, two (2) staff members reviewed video footage from the Discovery Classroom recorded on 03/30/2026, and determined that a staff member failed to wash children's hands after diapering and toileting within the classroom. General: Based on interview, two (2) staff members reviewed video footage from the Discovery Classroom recorded on 03/30/26, and determined that a staff member failed to wash to was her hands after assisting the children with diapering and toileting.

One (1) staff member (DOH: 12/01/25) did not have documentation of a negative TB test or a statement from a healthcare professional stating staff is free from active tuberculosis on file at the facility. 2. One (1) staff member (DOH: 12/01/25) had a TB skin test dated 09/03/23.
