General: Based on observation, interview, and review of documentation, it was found that a teacher inappropriately handled a one-year-old child by the wrists and got into the child's face.
General: Based on observation and interview, there was a teacher's cell phone accessible to the children in care within the Early Preschool Classroom. The cell phone was located on the edge of the countertop in the room. During the exit interview, the staff-in-charge indicated that the staff members use their cell phones to update the ProCare app throughout the day.
General: Based on observation and interview, there was a cabinet within the restroom located across the hall from the Pre-K Classroom, this cabinet was missing one (1) of the doors. During the exit interview, the staff-in-charge acknowledged that this door was missing. The staff-in-charge reported that the door had been in disrepair but then was completely taken off by a maintenance person to be repaired; however, the door has never been fixed and replaced by the maintenance person.
General: Based on observation, the Toddler #1 Classroom contained two (2) stained ceiling tiles. These were located in the middle of the classroom.

General: Based on observation during a tour of the facility, the following was found: 1) In the Early Preschool Room, a spray bottle with the words "bleach water" written on the side was stored in an open compartment on the diaper changing station. 2) In the boys' bathroom located in the hallway near the indoor gross motor room, a can of Oder Eliminator was stored in a low unlocked cabinet. The label on the can indicates to keep out of the reach of children. Both items were within reach of the children.

There was a red plastic side panel on the side close to the sand box that was cracked and broken where it was attached to a metal support bar. Colored duct tape had been used to repair the crack and was starting to fray at the edges. During a tour of the preschool classroom the surveyor observed a staff person washing sippy cups by hand in the classroom sink. General: Based on Review of Documentation, staff person hired 3/28/17 had the results of a criminal records check dated 3/30/17. There was no evidence on file that the application for the check had been submitted prior to employment as required.
General: Based on Observation, during a tour of the Toddler II room a water stained ceiling tile was observed near one (1) of the light fixtures. During an interview with the staff person in charge it was indicated that a leak had been found and that an order has been placed to replace the ceiling tile.
General: Based on Interview, with the staff person in charge it was found that the parents are responsible for providing lunch for their child each day. Lunch was observed being served in the Early Preschool room. The following individual lunches were observed that did not have the required components for lunch; corn, orange, bagel with cream cheese and watermelon fruit juice; pretzels, popcorn and strawberries; macaroni and cheese, yogurt and a roll. An interview with the staff person in the room found that if the child did not bring anything to drink or brought somthing other than milk they will offer them water to drink with their meal. An interview with the staff person in charge found that they had cheese sticks and bananas available for those children as a suppliment to those lunches that did not meet the requirements. However, the staff person in this room was not observed serving these items to those children whose lunches did not meet the requirements. Children's Records

General: Based on Observation, the surveyor observed a gallon of 2% milk in the refrigerator. It was revealed that children ages two (2) years old to school age are being served the 2% milk instead of 1% or skim milk. 990 - Bottle Preparation by Parent Not Applicable 922 KAR 2:120. Section 9. General: Based on observation, this regulatory requirement was not met. During a tour of the facility the following was observed. 1) A toilet plunger was found by the toilet in the first stall in the boy's bathroom located near the facility office. 2) A toilet plunger was found by the toilet in the first stall in the girls' bathroom located near the facility office. 3) In the Toddler II room there were large vinyl blocks in various shapes for children to walk and climb on. These blocks were observed in the corner of the room near a low wooden shelf. One (1) of the blocks, shaped like a triangle, was placed on the floor to form a ramp to the other blocks. Some of the children were observed running down the ramp. Due to the location of the end of the ramp and the position of the shelf there was not enough space to keep a child from hitting the shelf if they should happen to fall. A review of eleven (11) staff files found one (1) staff person with a hire date of 8/15/18 who had the results of a T.B. test read on 3/2/19. This date is after the employee's date of hire and not prior to the date of hire as stated in this regulation. There was no other documentation on file to verify that a T.B. test had been completed prior to employment.

During a tour of the Social Hall/Gross Motor Room the surveyor observed two (2) large containers (sixty- seven (67) ounces each) of Germ X hand sanitizer with pumps sitting on a table near the entrance/exit door. General: Based on Review of Documentation and ECE-TRIS records, there was one (1) staff person with a hire date of 8/23/00 who only had evidence of having completed fourteen (14) hours of training for the 2014-2015 training year. During an interview with the staff person who is also in charge of going through the staff files and assuring everything is current indicated that she was sure she had completed fifteen (15) hours of training. Driver history records through the state transportation agency that issued the driver’s license; (c) Hold a current driver’s license which has not been suspended or revoked during the last five (5) years; and (d) Not caused an accident which resulted in the death of a person. General: Based on Observation, during a tour of Toddler II there were six (6) children ages one (1) to two (2) years old in the room. An interview with staff found that three (3) of the children are two (2) years old. During an interview with the staff person in charge it was indicated that these children are enrolled in the classroom and remain in the room all day. The staff person in charge also indicated that there is not room to move the two (2) year old children into an older room. The staff person in charge stated further becasue they operate during the school year only she did not want to move the children to another room for only a few months. There was not a transition plan for these children. 420 - Infant Sleep Position Not Applicable 922 KAR 2:120. Section 6. Sleeping and Napping Requirements. (1) An infant shall sleep or nap on the infant’s back unless the infant’s health professional signs a waiver that states the infant requires an alternate sleeping position.
General: Based on Observation, at the time of the survey lunch was observed being served in the Early Preschool room. The children's drinks were observed lined up along the edge of the counter. Children were heard asking the staff person for something to drink and the staff were respond, "Just a momnet. Eat a few more bites please." During an interview with the staff person in charge it was indicated that the staff person wanted to make sure the children ate their food and did not fill up on liquid.
General: Based on Observation, during a tour of the preschool room several fish were observed in a fish tank. During an interview with the staff person in charge it was found that the fish had only been at the facility for approxiamtely one (1) month and they had not gotten written permission from the children's parents allowing the fish to be in the classroom.
General: Based on Review of Documentation, one (1) child (enrollment date: 7/29/11) had an immunization certificate that was no longer current as of 2/28/12.
