General: Based on observation, the surveyor found two (2) Pacific Blue surface wipes dispenser buckets on a window ledge. The surveyor noted the sanitizing wipes were within reach of the children in care. At approximately 3:11 p.m., a staff member walked to the window ledge and got some sanitizing wipes from one of the dispenser buckets. A school-age child followed the staff member and pulled a couple of wipes from the bucket. The staff member immediately took the wipes form the child and put them back into the container. General: Based on interview, this regulatory requirement was not met. A staff member inappropriately handled a child in care. The child reported to administrative staff that the teacher had placed him in a headlock, flipped him upside down, and patted his bottom. The staff member denied placing the child into a headlock but admitted to another staff member that she had turned him upside down and patted his bottom three times. This interaction caused discomfort to the child, despite the alleged playful manner in which it occurred.

This volunteer was observed supevising children, giving them instruction on where to go and what to do and had an evaluation on file with the title of After School Program Director dated 4/15/15. Further review of this volunteers file and ECE-Tris records found that there was no evidence of the completion of fifteen (15) hours of training for the 2015-2016 training year. During an interview with the staff person in charge it was acknowledged that this volunteer had not completed training and that she must have misunderstood what is considered a volunteer. There was evidence of this volunteer having completed training in previous years. A review of KARES records supports the information found during the survey. One of the staff persons was observed alone with a group of 16 school-age children.
General: Based on observation, there was one staff person supervising a group of 42 children from 2:50 p.m. to 2:59 p.m. in the cafeteria. The children ranged in age from 8 years to 12 years old. General: Based on Review of Documentation, there was one (1) staff person who was rehired 8/9/16 and did not have the results of a T.B. test read until 9/7/16. There was an additional staff person with a hire date of 2/6/17 who had a T.B. screening completed on 2/7/17. Both of these were completed after the staff person was hired. There was an additional person listed as a volunteer who had the results of a T.B. test read on 3/28/14. This exceeds the two (2) year minimum requirement for a T.B. test. This volunteer was observed supevising children, giving them instruction on where to go and what to do and had an evaluation on file with the title of After School Program Director. The facility also requires volunteers to have a T.B. skin test.
General: Based on Review of Documentation, there were four (4) staff whose last documented evaluation was dated 4/15/15. An interview with the staff person in charge found that evaluations for 2016 had been completed were kept on file in the main office located at the school and had not been placed in the file for the afterschool program. General: Based on Observation, the water temperature at the sink in both the girls and boys bathrooms registered one hundred, twenty-eight (128) degrees Fahrenheit when measured with a calibrated thermometer. The children use these sinks for hand washing.
General: Based on Review of Documentation, children enrolled 4-20-10 and 6-3-14 did not have evidence of a preferred hospital on file in the facility. The facility failed to have a written enrollment record for each child enrolled in the facility. There were three (3) separate sibling groups that shared an enrollment form. Each group listed two (2) children on the same form. Nine (9) of the ten childrens files reviewed by Surveyor did not have adequate identification information for each child; no date of birth noted on enrollment form. There was no evidence of enrollment dates on file in the facility for three (3) of the ten (10) children's files reviewed.
