General: Based on review of documentation, the following was found: 1. One child (DOE: 04/15/24) did not have a current immunization certificate on file. The certificate on file was dated 12/06/25; therefore, it was no longer current. 2. One child (DOE: 07/27/25) did not have a current immunization certificate on file. (1) A child-care center shall maintain: (b) A written record for each child: 1. Completed and signed by the child’s parent; 2. Retained on file on the first day the child attends the child-care center; and 3. To contain: a. Identifying information about the child, which includes, at a minimum, the child’s name, address, and date of birth; b. Contact information to enable a person in charge to contact the child’s: (i) Parent at the parent’s home or place of employment; (ii) Family physician; and (iii) Preferred hospital; c. The name of each person who is designated in writing to pick-up the child; d. The staff person stated that two other staff were supposed to be present but had not yet showed up to work. General: Based on observation and interview, this regulatory requirement was not met. The following was found: 1. The surveyor arrived at the facility at 9:00 AM and observed one staff person caring for three children who were eating breakfast. There were no other staff or children present in the building at the time. The surveyor asked the staff person how breakfast was prepared for the children. The staff person responded that she cooked the eggs and sausage in a skillet on the stove in the kitchen and further stated that the three children she had present at the time waited in the front entry way while she was cooking. The front entry way where the children were identified as being is not visible from the stove area in the kitchen; therefore, the staff person was not able to adequately supervise the children. 2. At 9:29 AM, the surveyor observed the only staff person present in the facility escort three children down the hallway from the preschool classroom to the infant classroom. The staff person stated that she was taking the children down the hall to wipe their face from breakfast. One child (five-year-old) remained sitting at the classroom table in the preschool classroom during this time. The distance from the preschool classroom to the infant classroom down the hall was approximately 20 feet. The five-year-old child was not visible from the infant classroom; therefore, adequate supervision was not provided. 3. At 9:33 AM, the surveyor observed the staff person escort the three children from the infant classroom, back through the front entry way, through a door that enters a hallway on the opposite side of the building and into the children's bathroom area. The door that leads into the hallway was closed while the staff person and the children were in the children's bathroom area; therefore, the five-year-old child was not visible from the children's bathroom area and adequate supervision was not provided. The staff person and the three children were in the children's bathroom area approximately four minutes. There was a clear spray bottle with a green substance in it sitting on top of a cabinet in the eating area with no label on it. A preschool child grabbed the bottle and began to spray it around the other children who were lined up to go to the bathroom. A teacher got the bottle from the child and placed it back on top of the cabinet where it came from. It could not be determined what was inside the bottle.

General: Based on Observation, there was one (1) infant present at the time of the survey. During the entire survey she was either carried around or placed in a high chair with nothing to do. The surveyor was at the facility from 8:30am to 10:40am. At one point the staff member pulled the child around in the high chair while she was getting paperwork for the surveyor. The infant was then taken on a walk for approximately thirty (30) minutes and then was brought back inside and was found in the church auditorium with the staff member with no activities for the child.
General: Based on Observation, there was a great deal of rocks, debris and pieces of paper were found in the shower basin in the boys' bathroom that the children use.

For approximately thirty (30) minutes the preschool teacher wore headphones while interacting and working with the children. It is not certain if he had music on at the time but he was heard raising his voice several times with the children.
General: Based on Observation, the surveyor found a staff member and an infant in the church auditorium with the lights off. The staff member was sitting in a chair with the child standing next to her. There were no activities for the child and this area is not licensed space. It could not be determined how long they were in the unlicensed space.
General: Based on Review of Documentation, there were no written annual evaulations completed on any staff member.
General: Based on Review of Documentation, staff member hired on 04/15 did not have a child abuse and neglect check on file or any documentation showing that the check had been submitted. This staff member was working alone with the children at the time of the survey. Staff member hired on 10/06/14 had a criminal records check dated 11/14/14. Substitute staff member hired on 05/08/15 did not have a criminal records check or child abuse and neglect check on file. She was not present the day of the survey.
General: Based on Review of Documentation, staff members hired on 02/02/14 and 05/08/15 did not have documentation from a health care professional stating they were free of active tuberculosis. Staff member hired on 10/06/14 had a TB skin test on file dated 10/21/14.

The staff member stated that the children never go to the back of the playground but it is still accessible to the children. On the side of the playground where the children mainly play there was a broken bucket, paint and a rusted metal For Sale sign all accessible to the children.
General: Based on Observation, an outlet cover was broken in the two (2) year old classroom. A blue bench in the two (2) year old classroom had a tear across the top and blue duct tape was covering the tear.
General: Based on Observation, there was no gross motor toys/equipment for the children on the outdoor playground.
General: Based on Observation, the only menu posted in the facility was found in the kitchen. Upon arrival to the facility the children were having sausage pancakes on a stick and milk to drink. The menu for breakfast that day stated toast, apples and orange juice would be served. When asked what the children would be served for lunch a staff member stated pizza rolls and tater tots. The surveyor looked in the freezer and there were no pizza rolls or tater tots. The menu for lunch that day stated chicken alfredo, peas, wheat bread and milk would be served for lunch.
General: Based on Observation, upon arrival to the facility, the children were eating sausage pancakes on a stick and had milk to drink. There was no fruit, vegetable or 100% juice served. The meal component was not met.
General: Based on Review of Documentation, the emergency preparedness plan was not on file.
General: Based on Review of Documentation, it was found that the facility was not following the conditions of their zoning permit. The facility was found to be operating at 6:30am. The zoning permit states that the facility can not being operating its daycare until 7:00am.
General: Based on Observation, it was found that the facility was not providing 1% milk for the children ages twenty-four (24) months to school age. There were obsered to be several cartons of 2% milk in the refridgerator.
General: Based on Observation, it was found that the infant teacher left three (3) infant children unsupervised in the infant classroom for approximately five (5) minutes while she walked up to the front of the building.

General: Based on Observation, there was a loaf of white bread and a loaf of wheat bread observed in the kitchen that were to be served to the children. The wheat bread's first ingredient was enriched bleached flour, not whole or enriched grain.

Two (2) children enrolled 5/13/12 and 10/15/12 did not have authorization by the parent for the child care center to seek emergency medical care for the child in the parent's absence. 3. Three (3) children enrolled 11/9/12 and one (1) child enrolled 10/15/12 did not have the name and phone number of a person to be contacted in case of an emergency.
