General: Based on observation and interview, it was determined this regulatory requirement was not met. During a tour of the One (1) and Two (2) year old room, the surveyor observed the high chairs used for food service to be stored in the toilet room. The surveyor specifically noted the chairs to be stacked in such a manner that one (1) of the chairs was located inside the toilet bowl. The teacher in the room stated the chairs were brought out at lunchtime and the toilet was used for potty training. The staff member stated the chairs were stacked back in the toilet room after lunch. Due to the use of the toilet by children, the chair could not be considered sanitary for use. The chair was located inside the toilet bowl and was stacked on top of other chairs. The surveyor would consider each of the chairs in proximity to the toilet bowl and those touching it to be unsanitary for food service. General: Based on observation, it was determined a six (6) month old child was sleeping on the floor when the surveyor entered the room. The child was lying on a small pillow on the floor. The teacher was feeding another child while the surveyor was in the room. The director who had entered the room with the surveyor picked the child up from the floor and held the child in her arms until the surveyor was ready to leave the room. The director stated the child, "had just fallen asleep". It was also determined that four (4) of the crib sheets in use in the infant room were stained and dirty. The sheets contained black and brown stains. The teacher stated the sheets were going to be washed that afternoon. General: Based on review of documentation, it was determined that staff members hired on 11/26/2012, 02/12/2014, 06/10/2014, and 05/18/2015, did not have evidence of completing 15 hours of annual training for the past year of employment in files presented for review. General: Based on Interview, a parent was notified on 12/1/17, by the facility staff in charge, that she needed to pick her child up from the daycare because a staff member was holding the child's hand to go outside and the child dropped to the floor hurting her arm. The staff stated that the child was crying and favoring her arm. The facility staff was notified by the child's parent on 12/1/17 (around nap-time when she picked the child up) that she was going to take the three (3) year-old child to the hospital to seek medical treatment because the child appeared to be in pain and she was favoring her arm. The facility owner called the Division of Regulated Child Care and self-reported the incident on 12/5/17. This was not within twenty-four (24) hours of when the parent notified the staff that she was seeking medical treatment for her child. General: Based on Observation, of a video recording on 12/1/17 a three (3) year-old child was grabbed by one (1) arm by a staff member resulting in the child having a dislocated elbow. The child had to seek medical treatment to put the elbow back in place. The video recording did not show the same information as to what the staff member wrote in the injury or accident report dated 12/1/17. The staff member indicated in the injury or accident report that the child was pushing another child as they were standing in line to go outside. The video recording did not show the three (3) year-old push a child and the children were not in line to go outside. The video recording showed the children getting their coats on to go outside and the child was standing around the teacher and the teacher grabbed the child by one (1) arm and lifted her up and then put her down. The injury or accident report further states that the child was taken to the office when she continued to cry. The video recording showed the class line up and go outside and a few minutes later the child was taken inside to the office. The facility terminated the staff member's employment. General: Based on Observation, it was found that the statements of deficiencies issued and the plans of correction submitted for the relicensure survey conducted on 8-18-16 and an allegation investigation conducted on 4-4-17 were not posted in the facility. This employee was not found to be working alone with children. She has also been employed for more than ninety (90) days. An employee hired on 3-15-17 was found to have lived in Florida within the last five (5) years. The employee's file did not contain evidence of a Florida background check being completed. This employee was observed to be left alone while supervising children in the school age classroom for approximately two (2) minutes.
General: Based on Observation, it was found that in order for parents and staff to reach the Older Ones classroom and the Twos classroom, they had to walk through the Younger Ones classroom. General: Based on Interview and review of documentation, it was determined that a two-year-old child was left unsupervied on the facility's playground for an undetermined amount of time. In addition, it was found that the child was signed in at the facility at 9:30 a.m. by the van driver on 3/27/17 but no one remembers seeing the child until he was found on the playground around 11:10 a.m. The Director stated that the staff person/van driver that signed the child in told her that he thought he took the child to his assigned classroom. The Director indicated that the staff person/van driver then said, "Maybe he followed me and his brother to (Threes & Fours) room." No one in the facility could indicate where the child was from 9:30 a.m. until he was found on the playground around 11:10 a.m. The Threes & Fours classroom is scheduled to be on the playground from 10:30a.m. to 11:00 a.m. on a daily basis. General: Based on observation, the surveyor found the following: (1) Five (5) overhead lights were not working in the Toddler #1 Classroom. (2) Eight (8) overhead lights were not working in the Toddler #2 Classroom. The diaper changing area in this classroom was dimly lit due to most of the lights not working. The owner stated that approximately six (6) months ago; they removed half of the wall that divides the Toddler classrooms and when doing so, the electrical wires to the lights were cut. He stated that the owner of the property has been informed about the issue. (3) Three (3) overhead lights were not working in the School Age Classroom. (4) One (1) overhead fluorescent light located in an area where the cots are stored was not working.

General: Based on Interview, it was found that in June of 2014 a parent accused one (1) of the facility's employees of touching her three (3) year old daughter in her private area. The parent spoke to the director about her concerns. The facility did not make a report to the cabinet as required. Director Requirements
General: Based on Observation, at the time of the visit conducted on 10/21/14 two (2) of the facility vans with children on board arrived at approximately 8:50 a.m. On one (1) van there were five (5) children ages infant to preschool and the driver. There was not another adult on the vehicle. A review of records found that the staff person driving the van alone with five (5) children had a Ohio driver's license on file. The director indicated that this staff person recently moved from Ohio and has not had her driver's licensed changed. In addition there was not a current driver history record on file through the stated transportation agency that issued the driver's license. There were two (2) staff on the other vehicle. A review of records found that the staff person driving (hired 10/20/14) did not have a copy of a driver history record through the state transportation agency that issued the driver's license. General: Based on Observation, at the time of the visit conducted on 10/21/14 two (2) of the facility vans with children on board arrived at approximately 8:50 a.m. On one (1) van there were five (5) children ages infant to preschool and the driver. There was not another adult on the vehicle.
General: Based on Observation, at the time of the visit on 10/21/14 a staff person hired 10/20/14 was observed driving one (1) of the facility vehicles. A review of records found there was no results of a criminal records check or the results of a child abuse and neglect check nor was there documentation to verify that the application for the results of both of the records check had been submitted. This staff person was not observed alone with children at the time of the visit.

General: Based on Observation, during a tour of the toddler room there were seven (7) one (1) year old children with one (1) staff person.
General: Based on Observation, there were three (3) unlabeled bottles and one (1) uncovered bottle in the refrigerator located in the blue, younger infant room.
General: Based on Observation, at the time of the survey the staff person in the infant room was observed feeding a child a bottle. She put the child down to put two (2) other children in their cribs. She then got another child who was awake out of a crib and sat the child in a high chair. She then gave the child a bottle and then went back and picked up the child she had been holding and feeding a bottle and continued feeding the child his bottle. The staff person did not wash her hands before giving the child she put in the high chair his bottle nor did she wash her hands before she continued feeding the child she was holding and feeding a bottle.
General: Based on Review of Documentation, there was one (1) staff person hired 8/12/13 who lived in Georgia within the last five (5) years. There was no documentation on file to verify that an application for the results of a criminal records check had been submitted for the state of Georgia.
General: Based on Observation, upon entry into the two (2) year old room there were nine (9) children visible but there was not a staff person. After being in the room ro approximately one (1) minute a staff person came out of the bathroom located in the room. The door was partially open. The staff person indicated she was helping a child who was in the bathroom. The child then came out of the bathroom once the staff person came out. The children who were in the room alone were not in scope of vision of the staff person in the bathroom.

General: Based on Review of Documentation, it was found that no one in the facility from open to close has current CPR and First Aid certification. It was also found that the van driver who transports children on a dialy basis does not have CPR and First Aid certification.

The dates of hire for these individuals were 06/03/2013, 06/03/2013 and 04/22/2013. It was also found that one (1) staff file out of nine (9) staff files reviewed had a TB skin test that was read past their date of hire. During a tour of the Two (2) year old room, the surveyor noted the lesson plan posted did not specify a date for when the plan was to be followed or implemented. General: Based on observation, it was determined in the Twos and Threes (2s and 3s) Room, the lights were not working in both fixtures on the left side of the room upon entering the space. The lights were in place, but would not function properly. It was also determined the light fixture was not working in the toilet room of the Two (2) year old room. The fixture did contain a bulb, but would not light when the switch was turned on. During interview with the Director and one (1) of the drivers it was found that the drivers will transport children by themselves if the numbers are low.

General: Based on Observation, it was found that an employee hired on 3-15-17 was observed to be left alone while supervising children in the school age classroom for approximately two (2) minutes. This employee was found to have lived in Florida within the last five (5) years. The employee's file did not contain evidence of a Florida background check being completed. General: Based on Review of Documentation, it was found that the files for employees hired on 5-18-15 and 6-15-15 contained the results of TB skin tests that were read on 5-12-15 and 6-12-15. These dates are not within the last two (2) years as required by the regulation.
General: Based on Review of Documentation, it was found that the file for an employee hired on 6-10-14 contained evidence of zero (0) hours of training being completed during the last completed training year. The file for an employee hired on 7-1-15 contained evidence of only five (5) additional hours of training being completed during the first year of employment. General: Based on Observation, it was found that the carpet in the Green Preschool classroom was frayed on the right side of the classroom. General: Based on Observation, on the large covered playground, it was found that the front panel of a Rheem air conditioning unit was open, exposing the mechanics inside. The front panel bore a label that stated, "Hazardous Voltage" "Can cause severe personal injury or death." This air conditioning unit is not fenced in and is accessible to the children. General: Based on Observation, it was found that an eight-month-old child was sitting on the floor while drinking a bottle.

General: Based on Observation, it was found that there were no thermometers in either of the facility's freezers. The cook searched the freezers during the survey and was unable to locate them.
General: Based on Review of Documentation, it was found that only twenty-five (25) of the fifty-four (54) children present were signed in to the facility. It was also found that several parents had already signed the end of the week verification space. One teacher was found to have signed her niece in to the facility at 7:00 a.m., however, the child did not arrive at the facility until after 10:00 a.m.
General: Based on Review of Documentation, the facility failed to submit criminal records checks in accordance to KRS 17.165 which requires the criminal records check be submitted on or before the first day of employment. One (1) staff file had a hire date listed as 2/22/16; however, the criminal records check was not requested until 2/29/16. Two (2) staff files had a hire date listed as 8/8/16; however, the criminal records checks were not requested until 8/12/16. Finally, there was no criminal records check documentation on file for a staff person hired on 8/15/16. Therefore, the surveyor could not determine when the criminal records check was requested for this individual.
General: Based on Review of Documentation, three (3) staff identified as van drivers had driving history records on file that were older than twelve (12) months. Programming
General: Based on observation at approximately 10:25 a.m., the surveyor entered the infant classroom and observed three (3) female toddlers sitting in their high chairs. The surveyor noted that the children did not have any food or any toys on their trays. One (1) child was crying and rubbing her eyes. A staff person stated that they were waiting for lunch to arrive. The surveyor asked how long the children had been sitting in their high chairs. The staff person stated, "Only five (5) minutes." At 10:30 a.m., the children were still sitting in their high chairs. At 10:32, the child that was crying was finally removed from her high chair and was comforted by a staff person. The children's lunch arrived at 10:35 a.m. The children were confined to their high chairs for approximately ten (10) to fifteen (15) minutes while waiting for lunch to arrive.
General: Based on observation, the surveyor observed an exhaust vent in the restroom located in the Two (2) year old classroom that contained an accumulation of dust.
General: Based on observation, the surveyor noted that in order to get to the Two (2) year old classroom, one has to walk through the Toddler classroom. A staff person in the Two (2) year old classroom stated that parents walk through the Toddler Classroom to drop off their children into the Two (2) year old classroom. She stated that the Two (2) year old classroom moved to this location in the building a couple of months ago. The Director stated that parents drop off their two (2) year old children in the computer room and then a staff person takes the children to the Two (2) year old classroom. Either way, the Toddler room is used as a walk through to another room.
General: Based on observation, the water temperature at the hand-washing sinks located in the Toddler #1 classroom, Toddler #2 classroom, and the Two (2) year old classroom registered at 130 degrees Fahrenheit.
General: Based on Interview and observation, the surveyor found that the staff are mixing bottles for two (2) children enrolled in the infant classroom. The surveyor observed two (2) cans of formula on the shelf. The staff person stated that she mixes the formula with tap water.
General: Based on Observation, it was found that the water temperature in the orange bathroom used for hand washing registered 132 degrees Fahrenheit.
General: Based on Review of Documentation, two (2) children's files were missing a phone number to contact the physician. In addition, one of the same children was missing a preferred hospital. This area was left blank on the child's enrollment form.
General: Based on Review of Documentation, two (2) staff files were missing professional development plans. Another staff file contained a professional development plan that was not dated. These staff were hired on 11/26/12, 5/8/14 (sub), and 2/12/14.
General: Based on Review of Documentation, monthly evacuation drills have not been conducted and documented.
General: Based on Review of Documentation, there was one (1) staff person hired 4/8/14 who had submitted a high school transcript as documentation of verification of his graduating from high school. However, the transcript does not include a gradution date to verify that the staff person completed the course of study required to graduate.
General: Based on Review of Documentation, there was not professional development plans for two (2) staff who had been employed at least one (1) year at the facility.
General: Based on Observation, there was one (1) child enrolled 12/21/13 who did not have an enrollment application. The child shared an application with a sibling. There was one (1) child who did not have a preferred hospital listed and three (3) children who did not have the name and number of the family physician listed. There was an additional child who did not have a preferred hosptial and the name and number of the family physician.
General: Based on Observation, there was not a daily planned program of activities posted in the pink, older infant room. It was also found that there was not a date listed on the planned program of activities posted in the blue, younger infant room.
General: Based on Observation, a staff person in the blue, younger infant room placed an infant in a crib on her stomach. The infant had a pacifier in her mouth that was attached to a cord.
General: Based on Observation, there was an area of the ceiling over the toilet in the orange bathroom located in the orange preschool room that was cracked. The paint around that crack was bubbled up.
General: Based on Review of Documentation, at the time of the survey the copy of the regulations available for staff had been printed 6/6/13. Additional changes have been made to some of the regulations since this date.
General: Based on Review of Documentation, there was one (1) staff person hired 11/7/12 who did not have evidence of having an annual evalution on file.
General: Based on Review of Documentation, it was found that staff who transport children do not have a driver history record through the state transportation agency that issued the driver's license. Programming
General: Based on Observation, upon arrival at the facility at approximately 9:10 a.m. fifteen (15) school age children were observed playing outside on the playground. The children came inside while the surveyor did an initial walk through to count children. This began at approximately 9:15 a.m. When the surveyor entered the room the children were sitting at tables. The school age staff person was cleaning a child's knee and putting a band aid on a cut she had received. The surveyor left the room and came back at approximately 10:00 a.m. The children were still sitting at the table. They were not engaged in any activity. Two (2) children were put in "time out" due to behavior. The staff person was observed doing paperwork at the time.
General: Based on Observation, a staff person in the blue, younger infant room placed an infant in a crib on her stomach. In addition the infant had a pacifier in her mouth. The pacifier was attached to a cord.
General: Based on Observation, a wall vent in the blue hallway was observed to be loose and pulled away from the wall. The vent also had an accumulation of dust, dirt and grime. In the four (4) to five (5) year old room a black door that leads to the outside had a white substance splattered all over the door. A white door located in the older infant room had a brown substance splattered all over it. The area around the ceiling vents in the orange preschool room had an accumulation of dust.
General: Based on Observation, it was found that you must go through the blue, younger infant room to get to the pink, older infant room and the one (1) year old room. This is the direct path in each room. There are outside doors to the pink older infant room and the one (1) year old room. However, those doors remain locked and staff and parents access those rooms by going through the blue younger infant room.
General: Based on Observation, at the time of the survey the staff person in the infant room was observed feeding a child a bottle. She put the child down to put two (2) other children in their cribs. She then got another child who was awake out of a crib and sat the child in a high chair. She then gave the child a bottle and then went back and picked up the child she had been holding and feeding a bottle and continued feeding the child his bottle. The staff person did not wash either child's hands before giving them their bottles.
General: Based on Observation, there was one (1) crib in which the mattress did not fit flat in the bed. The sheet made the edges of the mattress "bow up" leaving gaps around the edges.
General: Based on Observation, during a tour of the green three (3) to five (5) year old room a blue and white shelf was observed that had tape residue with an accumulation of dirt and grime.
General: Based on Review of Documentation, it was found that a daily inspection of the vehicle is not being conducted on either vehicle used for transportation. The facility transports child daily to and from the facility.
General: Based on Observation, ten (10) cups of milk were observed sitting on a tray in the refrigerator located in the kitchen. The cups of milk did not have a lid or protective covering over top of them.
General: Based on Review of Documentation, the last documented date the facility's Evacuation plan in the event of fire, natural disaster or other threatening situation that may pose a health or safety hazard to a child in care was submitted to the proper agency was 11/7/12.
General: Based on Review of Documentation, it was found that earthquake drills are being conducted every six (6) months and not quarterly. It was also found that a drill labeled "weather related" had been conducted every six (6) months. There was no indication that a tornado drill had been conducted quarterly with the date, time and names of children who participated.

General: Based on Observation, it was found that the statement of deficiencies issued by the cabinet for a survey conducted on 6/5/13 was not posted nor was the plan of correction submitted by the facility posted. In addition information regarding the Kentucky Consumer Product Safety Program and the program's website and information were not posted.
General: Based on Review of Documentation, it was found that the facility did not have evidence of routes taken for transportation.
General: Based on Observation, it was found that the baseboards around the floor in the school age bathrrom were chipped and stained.
General: Based on Observation, it was found that a bottle of A&D ointment was sitting on a shelf in the bathroom in the one (1) year old classroom. Outdoor Play Area
General: Based on Observation, it was found that the facility had six (6) non-compliant cribs that did not meet the Consumer Product Safety Commission Standards established in 16 C.F.R. 1219-1220.
General: Based on Observation, it was found that the staff in charge had her four (4) year old daughter lying down in the front office with the door pulled closed and no supervision. It was found that two (2) children in the school age classroom left the area with no supervision. One (1) child was observed to walking across the hall into the computer area and another child was observed to be sitting down against the wall outside of the school age classroom. In the preschool classroom, six (6) children were obsreved to be left alone while the preschool staff walked a toddler child into her classroom. In the younger infant classroom, five (5) infant children were left unsupervised while the staff went to the middle room where the sink was located to wash her hands. In the older infant classroom, four (4) infant children were left unsupervised while the staff went to the middle room where the sink was located to wash her hands.
General: Based on Review of Documentation, it was found that the facility did not have evidence of current state regulations for staff.
General: Based on Review of Documentation, it was found that four (4) staff files out of nine (9) staff files reviewed did not have evidence of a child abuse and neglect check. These four (4) staff were observed to be working alone with children. The staff person with a hire date of 06/03/2013 was observed to be working alone in the older infant classroom with four (4) infants. A staff person with a hire date of 06/03/2013 was observed to be working alone in the school age classroom with twelve (12) children. A staff person with a hire date of 03/28/2013 was observed to be working alone with four (4) younger infant children and a staff person with a hire date of 05/08/2013 was observed to be working alone with twekve (12) school age children. It was also found that three (3) staff files out of nine (9) staff files reviewed did not have evidence of criminal records checks and were observed to be working alone. The staffs' dates of employment were two (2) with a hire date of 06/03/2013 and one (1) on 03/28/2013.
General: Based on Review of Documentation, it was found that two (2) staff files out of nine (9) staff files reviewed did not have evidence of proof of education. The employees dates of hire were both 06/03/2013.

General: Based on Observation, it was found that there was one (1) uncovered bottle in the refrigerator in the toddler (1 year old) classroom.
General: Based on Observation, it was found that a bag of frozen chicken was sitting on top of the freezor being thawed.
General: Based on Observation, it was found that there was no diapering procedure posted in the younger infant classroom.
