General: Based on observation, the sink temperature within the Blue Jays Classroom did not exceed eighty-five (85) degrees Fahrenheit when measured with a thermometer.

General: Based on observation, the following was found: Hummingbird Hall Restroom 1. An accumulation of dust was on the ceiling vent in the restroom over the sink. 2. Stained ceiling tile was observed over one (1) of the toilets. Canary Hall Restroom 1. Stained ceiling tile was observed over one (1) of the toilets. Ladybugs and Grasshoppers Restroom 1. A hole was observed in the ceiling above one (1) of the toilets. It appeared that a ceiling vent was missing. Gym 1. Some stained ceiling tiles were observed in this area. One of the ceiling tiles was in disrepair and insulation was exposed from the ceiling. 2. An active water leak was observed from the ceiling of the gym during the inspection. General: Based on interviews and review of documentation, the child-care center failed to provide adequate supervision of a child in care. The surveyor and DCBS representative conducted interviews with staff and the child-care center director on 11/26/18. The director stated a five-year-old child who has cerebral palsy and is non-verbal was left alone on the playground on 11/21/18 for approximately fifteen (15) minutes before a staff person who was returning from lunch found her. The director stated a review of the security cameras found at approximately 2:02 p.m., the children in the blue and orange groups and two (2) staff enter the building after leaving the playground. She stated a review of the security cameras found at approximately 2:19 p.m., a staff person who was returning from lunch enter the building with the five-year-old child walking next to her. She stated the child was supposed to be with the children in the blue group. She stated that she spoke to the child’s teacher who admitted that he did not check his group list prior to entering the building. She stated the staff person was terminated that day for violating safety policies. The staff person who found the child stated she returned from lunch at approximately 2:07 p.m. and parked her car facing the playground. She stated that she noticed the child standing next to the gate crying. She stated, “She was crying and pretty hysterical.” She stated once the child saw her, she calmed down and stopped crying. An interview conducted on 12/3/18 with the staff person who had been responsible for the child indicated that the child must have gotten out of line at some point and hid. He admitted that he did not realize she had been left on the playground. ECE-TRIS confirmed this. It was also found that a staff person with a hire date of 11/10/2015 did not complete orientation training until 03/22/2016. The staff in charge acknowledged this.

General: Based on Observation, during a tour of room 116 six (6) sippy cups of milk were observed in the refrigerator. An interview with staff found that the milk was left over from the morning snack and will be reserved with lunch.
General: Based on Observation, during a tour of room 111 there were seven (7) containers of diaper ointment and a packet of Butt - Paste in an unlocked box on a shelf on the wall where the diaper changing table is located. Drug facts were listed on the back of each container and the packet of Butt-Paste was not labled for an individual child. During a tour of room 112 eleven (11) tubes of diaper ointment were found in an unlocked box on a shelf located on the wall by the diaper changing table. Drug facts were listed on the back of each of these tubes of ointment. During a tour of room 116 there were two (2) containers of diaper ointment sitting on a shelf located on the wall near the diaper changing table. Drug facts were listed on the back of the these containers. An inspection of the first aid kit found a bottle of Kroger brand Children's Pain Relief, Aquaphor Healing Ointment, Neosporin Ointment, Children's Ibuprofen, CVS brand Cortisone Cream and a bottle of acetamenophen that were not labeled for an individual child. Drug facts were listed on the back of all of the items. A bottle of Children's 24 hour Allergy Medicine was found in the door of a refrigerator in a small room that leads to the staff break room. General: Based on observation, the surveyor entered the cafeteria and found several ceiling tiles that were in disrepair. The surveyor observed water stains, discoloration, and black residue covering many of the tiles. Additionally, there were three missing tiles and a couple of tiles that were not secured in place. The staff person in charge stated the damage was from the condensation from the air conditioning system. It is important to note, that the cafeteria space is used as a school-age classroom each day. The surveyor also found a ceiling tile not secured in place over the diaper-changing table located in classroom #115. General: Based on interviews and review of documentation, the child-care center failed to provide for the health, safety, and comfort of a child in care. The surveyor and DCBS representative conducted interviews with staff and the child-care center director on 11/26/18. The director stated a five-year-old child who has cerebral palsy and is non-verbal was left alone on the playground on 11/21/18 for approximately fifteen (15) minutes before a staff person who was returning from lunch found her. The director stated a review of the security cameras found at approximately 2:02 p.m., the children in the blue and orange groups and two (2) staff enter the building after leaving the playground. She stated a review of the security cameras found at approximately 2:19 p.m., a staff person who was returning from lunch enter the building with the five-year-old child walking next to her. She stated the child was supposed to be with the children in the blue group. She stated that she spoke to the child’s teacher who admitted that he did not check his group list prior to entering the building. She stated the staff person was terminated that day for violating safety policies. The staff person who found the child stated she returned from lunch at approximately 2:07 p.m. and parked her car facing the playground. She stated that she noticed the child standing next to the gate crying. She stated, “She was crying and pretty hysterical.” She stated once the child saw her, she calmed down and stopped crying. An interview conducted on 12/3/18 with the staff person who had been responsible for the child indicated that the child must have gotten out of line at some point and hid. General: Based on Observation, in the Doves room there were eight (8) red and blue tri-fold nap mats stacked on top of cots. The mats had numerous tears around the edges that exposed the foam inside. Repairs to the mats had been made using silver and red duct tape. An interview with staff in the room found that mats are used to stand on their sides to create a barrier between children at nap time because some of the children like to talk to the child beside them instead of resting. A staff person with a hire date of 11/23/2015 did not receive a completed TB skin test until 04/05/2016 and another staff person with a hire date of 09/14/2015 did not receive a completed TB skin test until 12/18/2015. General: Based on Observation, in a freezer located in a refrigerator in a small room where the staff break room is located five (5) packets of breast milk were found. There was not a thermometer in the freezer.

General: Based on Observation, it was found that the facility menu is posted on a cabinet in the cafeteria. An interview with the director found that only staff and some children are in the cafeteria. This is not a conspicuous place.

General: Based on Observation, during a tour of the Room 113 there were numerous areas of chipping and peeling paint on the wall by the door. In rooms 108, 105, 103, 102 and 100 the ceiling vents had a large accumulation of dust. In the bathroom in the preschool hallway there was a hole in the ceiling over one (1) of the bathroom stalls where a ceiling vent had been. In this same bathroom there were seven (7) water stained ceiling tiles. In room 104 and 102 there were water stained ceiling tiles.
General: Based on Observation, during a tour of room 113 the front cover of the heating vent had come loose. During a tour of the bathrooms located in the hallways and in room 102 the steps stools located at the sinks were found to be embedded with dirt and grime.
General: Based on Review of Documentation and interview, the facility failed to meet this regulatory requirement. A review of the attendance records during the inspection and an interview with the staff person in charge found that an administrative office staff person documents daily arrival and departure times for children on the required form provided by the Child Care Assistance Program; however, the administrative staff person is not having the child's parent verify the information for accuracy each week as required. None of the January - March 2014 attendance records contained parent signatures and/or initials.
General: Based on Interview, it was found that there was no documentation of current liability insurance at the facility. The staff person in charge stated that the staff person that had this documentation was on vacation; therefore, it was not available for review.
General: Based on Interview, the facility failed to meet this regulatory requirement. During a tour of the facility the staff person in charge identified a floor to ceiling wall with a door that was installed to divide an area into two (2) separate classrooms (Grasshopper #1 and Grasshopper #2). The staff person in charge stated that she obtained proper permits prior to construction; however, did not have this documentation available during the survey. Furthermore, the facility failed to contact/notify the cabinet prior to this construction project.

General: Based on Observation, the following was found: 1. A radio was observed sitting in a window ledge in Room 112 (Caterpillars). The radio was plugged into an electrical outlet a few feet away and the cord was observed hanging loose. Cribs were positioned in the same area. 2. Two (2) radios were observed sitting in window ledges in Room 111 (Roly Poly's). The radios were plugged into electrical outlet several feet away and the cords were observed hanging loose. Cribs were positioned in the same area. 3. During a tour of Room 112 (Caterpillars), the surveyor observed a one (1) year old child sitting on the tile floor while drinking from a bottle. This is not a sanitary area for a child to consume a meal.
General: Based on Observation, the facility failed to ensure an opening to the outside be protected against the entrance of vermin. During a tour of the facility, the surveyor observed a window open in Room 116 (Butterfly) and the window did not contain a screen.

The menu in the Ladybug classroom stated that lunch was at 11:30am and snack was at 3:00pm. The menu in the Grasshoppers classroom stated that lunch was served at 11:30am and snack was served at 3:00pm. Many of the meals did not indicate an end time and some of the menu times exceeded three (3) hours.
General: Based on Review of Documentation, staff with hire date 8/9/12 did not have a criminal records check (CRC) in her file and available to review within the facility. Staff with hire date 8/15/12 did not have a CRC submitted until 8/23/12.
