General: Based on review of documentation, the following was found: 1) A staff member (DOH 07/14/23) did not have an annual evaluation on file. 2) A staff member (DOH 07/26/23) did not have an annual evaluation on file. 3) Three (3) staff members (DOH 08/21/23) did not have an annual evaluation on file.
General: Based on interview and review of documentation, the surveyor noted that there were no staff members certified in First Aid and Cardiopulmonary Resuscitation (CPR) present during the afterschool program hours of operation (2:45 pm to 6:00 pm) on the day of the renewal inspection. The only staff member with First Aid and CPR certification was not present at that time.
General: Based on observation, the following was found: 1.) One (1) staff member (DOH: 07/26/23) only completed one (1) only of the required fifteen (15) hours of annual early care and education training for the 07/01/23 to 06/30/24 training year. 2.) One (1) staff member (DOH: 07/26/23) last completed pediatric abusive head trauma (PAHT) training on 02/27/19; however, did not renew the training due 02/27/24. 3.) One (1) staff member (DOH: 07/26/23) completed zero (0) out of the required fifteen (15) annual early care and education training for the 07/01/23 to 06/30/24 training year. 4.) One staff member (DOH: 08/21/23) only completed five (5) of the required fifteen (15) hours of annual early care and education training for the 07/01/23 to 06/30/24 training year. 5.) One staff member (DOH: 08/22/23) only completed fourteen and a half (14.5) of the required fifteen (15) hours of annual early care and education training for the 07/01/23 to 06/30/24 training year. 6.) An additional staff member (DOH: 07/26/23) completed zero (0) out of the required fifteen (15) annual early care and education training for the 07/01/23 to 06/30/24 training year. General: Based on review of documentation, two (2) children did not have current immunization certificates on file during the time of inspection.
General: Based on review of documentation, the following was found: 1.) One (1) children's file reviewed did not contain a preferred hospital. 2.) None of the children's files reviewed contained authorization by the parent for the child-care center to seek emergency medical care for the child. General: Based on review of documentation, the liability insurance that was on file at the facility was no longer current as of 07/01/24. Director Requirements In Compliance Employee Records Not In Compliance
General: Based on observation, the following was found: 1.) Three (3) staff members (DOH: 12/04/24, 12/10/24, and 07/26/23) did not have a TB skin test or a statement from a health professional on file at the time of inspection. 2.) Two (2) staff members (DOH: 08/14/24 and 08/13/24) did not have proof of a negative TB skin test or a statement from a health professional on file on of before their date of hire. The staff member who was hired on 08/14/24 had TB documentation that was dated for 09/15/24, and the staff member who was hired on 08/13/24, had TB documentation that was dated for 08/16/24.
General: Based on review of documentation, one (1) child's (DOE: 12/13/24) file reviewed did not contain information regarding the child's medical history and preferred hospital. It should be noted that all of the other children's files reviewed contained a separate document that listed these items, but this child's file was missing the document completely.
