A parent or guardian provides a written feeding statement listing the foods and drinks that an infant eats or drinks, including specific formula or breast milk. This schedule must be updated monthly and as needed.
A licensee shall keep a log that documents the printed names of the child and parents or guardians and the date the Parents Right to Know Form was provided. This information shall be included for children who do not become enrolled and those who are currently enrolled.
On May 6, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding positive behavior supports, diapering, lack of supervision, safety hazards, and not meeting the needs or supervision of an infant to ensure safety and capacity. An investigation began on May 7, 2025, and concluded on May 12, 2025. The complaint is substantiated for not meeting the needs or supervision of an infant to ensure safety because the infant, who was not capable of rolling over, was asleep on his stomach, on a couch, and on a boppy, and Provider #1 stated she was on a video call. The pack-and-play that was available had a burp towel, loose-fitting sheet, plastic toy, and a small stuffed animal attached to the pacifier placed inside. The complaint is substantiated for diapering because Provider #1 did not know when the infant's last diaper change occurred and was not documenting the diaper changes. However, Provider #1 stated the infant is often checked, but no definite time was provided. There is no evidence to substantiate the lack of supervision because Provider #1 stated the children outside the home were the neighbor s children. There is no evidence to substantiate the complaint for being over capacity because the attendance records indicated the family child care capacity maintaining the correct ratio, and the children outside were not children enrolled in the family child care. There is no evidence to substantiate safety hazards because the care areas had no hazards present. There is no evidence to substantiate improper positive behavior supports because the provider stated that time-out is only one minute per year and is used for children two and older. Additional concerns noted were infant safe sleep, the outdoor play gate was not self-closing and self-latching, providing care in an unlicensed portion of the home, child attendance, child files, infant daily record, infant feeding schedule, and bottles not labeled.

Complete a Parent's Right to Know Log.
Ensure child files are complete. Two children needed the date of enrollment hours and days attending, file copy of Parent's Right to Know, Screen Timer permissions, transportation permission, permission to sleep in other area. .
Ensure attendance is taken daily. This was corrected when the attendance was documented at the visit.
Ensure the fire extinguisher is mounted and inspected annually.
Ensure the freezer is at 0 degrees Fahrenheit. The correction was made during the visit and verified.
One annual training hours is needed.

Ensure attendance in electronic or written format is available.
Ensure the kitchen is clean. Clean or remove the dirty dishes from the sink, and counter area, and clean and sanitize the flat surfaces. Cover or remove the food from the refrigerator. Label food with the contents and a date opened or expiring. Add thermometers to the refrigerator and freezer.
Ensure hazards are removed or disposed of. A bucket with scissors and paint brushes is accessible to children. Ensure gates have self latching mechanisms.
Ensure the new construction/remodeled area is radon tested before seeking OCCL's approval to change care areas.
Ensure outlets are covered.
Ensure the indoor space that is inspected and approved by the fire marshal is used. LS McPhatter called Ms. Bulluck and it was disclosed that sometimes the children are using the newly renovated back room that was not fire marshal inspected. LS McPhatter informed Ms. Bulluck to use the front of the home that is OCCL and Fire Marshal approved.
Complete a Parent's Right To Know Log.
Ensure child files are complete. Child 1 needs a lead screening, child 8 needs a physical, immunizations, and lead and TB results, child 9 needs a physical, immunizations, TB and lead results.
Ensure children's attendance is completed daily with exact arrival and departure times.
Ensure to notify OCCL in five days and fire marshal for approval before changing rooms for care. The room must also have a radon test and be measure by OCCL. LS McPhatter observed that rooms for care are now in the living room and dinning room.
Complete two fire drills and fire safety inspections in March one will make up for February.
Ensure a completed renewal application is submitted 60 days before license expiration.

Ensure OCCL is notified of the change in care space. Upon arrival the provider was caring for children in the back room that is not licensed for care.
