Ensure that only certified staff administer medication. Ensure that medication administration records (MAR) are completed. Ensure creams are labeled with a child's name and are in their original containers. A staff member signed a medication log in December, and the administration of medication certification expired in October 2025. A child had two unmarked creams, and the MAR was incomplete, lacking medication names, doses, or start dates. One Child had vaseline in a clear plastic container with a red lid labeled with their name, and no MAR.

On April 7, 2025, the Office of Child Care Licensing received a complaint regarding not following accident/injury procedures, insufficient fall zone material, choking hazards accessible to toddlers, not following the facility termination policy, and not providing truthful information about the reason for termination. An investigation was conducted on April 11, 2025. This complaint is substantiated for a choking hazard accessible to a toddler because the administrator provided the cap that was removed immediately from the child s possession, and it was a choking hazard. There is no evidence to substantiate the allegation of not providing a truthful termination reason or following the facility termination policy because the facility handbook states immediate termination will occur if tuition is not paid for the week in care by Wednesday or if families and the administration cannot come to an agreement about a situation. There is no evidence to substantiate the allegation of insufficient fall zone material because the observed fall zones are covered as required. There is no evidence to substantiate the facility not following accident/injury procedures because the staff all stated proper accident/injury procedures, and it could not be confirmed that the injury happened at the facility after the child left for the day.

The Office of Child Care Licensing (OCCL) cannot legally disclose if a complaint was also received or investigated by any other agency. OCCL investigations of regulatory non-compliance begin when clearance from other agencies, when applicable, has been received. On June 24, 2024, the Office of Child Care Licensing received a complaint regarding improper discipline, not meeting the needs of the child, not providing a parent handbook upon enrollment, and not following the suspension policy. An investigation was conducted on July 1, 2024. This complaint is substantiated for not following their suspension policy because the current parent handbook did not state a child will be suspended for biting. There is no evidence to substantiate the allegation of improper discipline because Administrator #1 and Staff #1 stated no one ever hits children in care. There is no evidence to substantiate the allegation of not meeting the needs of the child because when staffing allows there is another staff member in the classroom, the child is shadowed, and the facility stated they have tried to have the parent complete an ASQ and a conference, but the parent did not complete the ASQ and did not show up for the conference. There is no evidence to substantiate the allegation of not providing the parent handbook upon enrollment because the child s enrollment date was February 14, 2022, and the parent signed a document stating she received the parent handbook on February 4, 2022. A concern noted was the facility did not follow proper accident/injury reporting procedures because there were some accident/injury reports that did not have a parent signature.

Child #7 needs days attending and TB results. Child #8 needs days attending. Child #9 needs date of enrollment, hours and days attending. Child #10 needs date of enrollment.

Ensure that exact arrival an departure of children is documented. LS McPhatter checked the sign in log in the Leaping Lions and 9 children were present in the room and 8 were signed in. This was corrected when the child was signed in to the classroom.
