Ensure to only throw soiled diapers in diaper pail. In the Two-year-old classroom, other garbage was observed int he diaper pail. ( corrected when the staff replaced the diaper pail liner)
The Office of Child Care Licensing (OCCL) received a complaint on August 8, 2023, regarding lack of supervision, not having working toilets in the center, and failure to notify OCCL within one business day of the breakdown. This complaint is substantiated because Ms. Reed, Staff #1, and Staff #2 stated a child was left outside alone while using the unapproved bathrooms due to not having working toilets inside of the center and for failing to notify OCCL as required of the equipment breakdown.
Disposition: Substantiated
On October 11, 2021, a complaint was accepted for investigation by OCCL regarding the center not following the suspension and expulsion policy, not modeling respectful communication, and not having a staff communication plan. An investigation was conducted on October 18, 2021, and concluded on October 27, 2021. This complaint is substantiated for not modeling respectful communication because Staff #1 stated she was arguing with a parent and raised her voice as the parent raised her voice. There is no evidence to substantiate the allegation of not following the suspension and expulsion policy because the center's parent handbook states child care will be terminated if a parent becomes hostile and cannot resolve the situation or for failure to pay for services as stated in the contract. There is no evidence to substantiate the allegation of not having a staff communication plan because all staff interviewed stated they communicate with each other before another staff takes over a classroom. A concern noted was a staff member was left alone with children without qualifications or a completed comprehensive background check.

On November 1, 2021, a complaint was accepted for investigation by OCCL regarding improper discipline, the classrooms not having activity areas, mouthed toys not being cleaned, not having a separate trash container for diapers, and the classrooms being too dark. LS Donovan and LSS Vandewater investigated the complaint on November 4, 2021. This complaint is substantiated for not having a separate trash can that is used exclusively for diapers. There is no evidence to substantiate the allegations of improper discipline because all staff members denied hearing a staff yell at children or taking away playtime as a punishment for not taking a nap. Staff also stated mouthed toys are washed and cleaned after a child is finished playing with them. During the investigation, LS Donovan and LSS Vandewater observed several activity areas in each classroom and the classrooms had sufficient lighting for supervision.
