On September 26, 2022, OCCL received a complaint regarding infant daily logs, diapering, infant feeding, staff-to-child ratios, and not allowing parents or guardians in the building. An investigation was conducted on September 26, 2022. The complaint is substantiated for incomplete infant daily logs because BrightWheel and the wall were not updated with diapering, feeding, sleep times, and sleep checks. The complaint is substantiated for staff-to-child ratios because LS McPhatter observed two staff present in the building with six infants, one six-year-old, and one three-year-old. When the LFCCH has more than four infants, there must be three staff present. The complaint is substantiated for improper diapering procedures because diaper checks were not documented hourly, diaper changes were not recorded, and proper diapering procedures were not followed during the visit. The complaint is substantiated for improper infant feeding because there was no preparation time and date on the infant bottles and three infants were not being held or placed in a feeding chair to be fed. All staff denied giving juice to infants. There is no evidence to substantiate the allegation of not allowing parents in the facility because staff stated parents are allowed in the building.

On September 14, 2022, OCCL received a complaint regarding improper discipline, lack of supervision, and accident or injury procedures. An investigation was conducted on September 19, 2022. The complaint is substantiated for lack of supervision because although the children were supervised, it was by Staff #2, an aide, who is not qualified to be alone with children. There is no evidence to substantiate the allegation of improper discipline because Staff #2 stated Child #1 got off the toilet once he became distraught and all staff interviewed denied improper discipline. There is no evidence to substantiate the allegation of improper accident and injury procedures because all staff denied observing marks on the children.
