Ensure cleaners are out of the reach of children. Specialist noted cleaners were kept in an plastic tote on the floor in the three-year-old room.
Ensure nap equipment is non-absorbent and easy to clean. Specialist noted several mats in the 3, 2 and 1 year old classrooms which were ripped and needed to be replaced. This was corrected when admin brought out several boxes of new mats to replace them with.
The Office of Child Care Licensing received a complaint on November 15, 2022, regarding not following the diapering procedure, improper sanitation, and not allowing parents into the building. The complaint is substantiated for not allowing parents into the building because the specialist observed several children getting dropped off at the foyer, and when asked, parents stated they had to stay in the foyer during drop-offs. There is no evidence to substantiate the complaint for not following the diapering procedure and improper sanitation because all staff had the proper knowledge of the diapering procedures, and the Specialist observed the center to be clean and in good condition.

OCCL received a complaint on February 2, 2022, regarding staff yelling at the children, which is under the positive behavior management regulation. An investigation was completed on February 7, 2022. The complaint is substantiated because two children stated sometimes their teacher yells. Also, Child #1 stated Staff #2 said to Child #2, If you do not stop crying, I will throw your shark away. Child #2 confirmed Staff #2 stated this. Child #1 also stated Staff #2 said, I do not care if you cry, after the Child #1 was placed in time-out. Ms. Morris and all other staff interviewed denied yelling at the children. In a written statement provided by owner Rodrick Morris, he stated the staff denied the allegations the children made during OCCL s investigation.

Ensure all staff files are complete. Staff #2 needs annual review of Abuse and Neglect.
Ensure all staff is eligible or provisionally eligible before starting employment. Staff #1 was present for her first day before being eligible. This was discussed and corrected at the visit.
Ensure there are sufficient toys and books. The Little Scholars classroom had two books and needs toys for the dramatic play area.
Ensure that children's hands are washed after toileting or having diapers changed. Children were observed using the toilet and not washing their hands. This was discussed and corrected at the visit.
Ensure child files are complete. Child #3 needs immunizations and blood lead screening results.
Ensure meals meet OCCL standards. Breakfast was observed with oatmeal and raisins, not the required side of fruit. This was discussed and corrected at the visit.
Ensure staffing ratio is followed at all times. Administrator Morris was present in the building with five other staff listed as either EC intern or EC assistant teacher.
Ensure all rest equipment is labeled.
Ensure application is submitted at least sixty days before license expires
Ensure there is enough toys/materials for the amount of children enrolled. One year old classroom needs more toys/materials.
Ensure child and staff's hands are washed after each diaper change, and the diaper changing mat is disinfected after each use. LS Smith observed staff #1 change two children with the same pair of gloves, and with out washing her or the child's hands. This was discussed and corrected at the visit.
Ensure fridge and freezers have working thermometers. Freezer in the kitchen, and fridge in the infant room needs a thermometer. Ensure food is labeled with contents and stored properly.
Ensure child files are complete. Child #3, #6, #7, and #8 need lead test results. Child #3 needs immunization records. Child #1, #2, #3, #4, #6, #7, #8, and #10 need developmental plans.
Ensure infants are held during feedings, or sitting in a feeding chair if they can hold their own bottle. LS Smith observed and infant laying flat on the ground while drinking a bottle. This was discussed with the teachers and corrected at the visit.
Ensure staff files are complete. Staff #1 needs CPR and First Aide. Staff #2 needs 2 references, health appraisal with TB results, completed or documented attempts for a service letter.
Ensure children's attendance is documented with departure and arrival times. LS Smith observed five children present, with only four signed in. This was corrected when the staff member signed the fifth child in.
Ensure child files are complete. Child #1 needs parents right to know, and screen time permission. Child #3 need updated health appraisal. Child #7 needs updated immunization record. Child #5, #6, #7, #8, and #10 need developmental plans.
Ensure infant sleep checks are completed an documented every 30 minutes.
