Ensure rest equipment is non-absorbent. Replace the ripped nap mats in the two-year-old classroom. Ensure safe sleep practices are used. A blanket was observed being used in place of a tightly fitted, fitted sheet in an infant's crib. An applesauce pouch lid was observed in a crib with a napping infant.
On January 28, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding not notifying OCCL of an equipment breakdown, not maintaining proper air temperature, security procedures, and not meeting the needs of the children. An investigation was conducted on January 28, 2026. The complaint is substantiated for not notifying OCCL of an equipment breakdown. According to Administrator #1, the building s heating system stopped working prior to opening on January 21, 2026. Administrator #1 failed to notify OCCL of the equipment breakdown. There is no evidence to substantiate for not maintaining proper air temperature, security procedures, or not meeting the needs of the children because staff denied the allegations and stated no children had been in care at the center since relocating them to another licensed child care center on January 21, 2026.

On September 2, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding handwashing procedures, not having soap and paper towels in the bathroom, and not following the schedule for naps. The complaint is substantiated for handwashing procedures because Staff #1 did not know there was hand soap in the soap dispenser on the wall in the infant classroom and was cleaning her hands and the infants hands with baby wipes. There is no evidence to substantiate for not having soap and paper towels and not following the schedule for naps because staff denied the allegations. LS #1 observed the hand soap and paper towels at each sink as well as additional supplies in a supply cabinet. LS #1 observed posted schedules that list nap time is from 12:00 pm to 2:00 pm. A concern noted during the visit was the two gates in the front of the building were observed not to be self-closing and self-latching. A second concern noted was the trash can in the bathroom located in the pre-k classroom was observed without a cover.

On January 21, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding a lack of supervision. An investigation was conducted on January 22, 2025. There is no evidence to substantiate for lack of supervision because Staff #1 stated she observed the incident occur but was too far from the children to avoid Child #1 receiving the injury.

Place a thermometer in the refrigerator and the freezer in the infant classroom. The thermometer in the freezer located in the kitchen pantry/closet read 26 degrees F. The large stand alone freezer in the kitchen did not have a thermometer. The freezer was not working properly and all the food inside was thawed. After discussion with staff, the freezer commonly freezes over and has to be thawed out frequently. The administrator stated the freezer was thawed out yesterday and all the food was transferred to the refrigerator to thaw the freezer. She stated the food was transferred back to the freezer the morning of the visit. After discussion with the center's CACFP representative, it was determined the food could be placed in the fridge and safe to use within the next 24 hours.

On July 9, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding room temperature. An investigation was conducted on July 10, 2024. There is no evidence to substantiate this complaint because when the supervisor conducted a visit in the afternoon, all classroom temperatures were below the maximum temperature of 82 degrees.
