On June 8, 2026, and June 22, 2026, the Office of Child Care Licensing (OCCL) received two complaints regarding a lack of positive behavior supports and a lack of supervision. An investigation began on June 16, 2026, and concluded on June 23, 2026. There is no evidence to substantiate for a lack of positive behavior supports because interviews with Administrator #1 and Staff #1 were consistent with the incident report provided by the child care center. Both individuals reported that Child #1 became upset, engaged in aggressive behavior toward Staff #1, and that the scratch occurred during Staff #1's efforts to protect themself from being hit. Administrator #1 reported that their review of the available video footage did not identify concerns regarding Staff #1's conduct and did not indicate malicious intent. The injury observed on Child #1 consisted of a superficial scratch on the forearm that appeared consistent with the explanation provided by Staff #1 that the injury resulted from contact with her fingernail during the interaction. There is no evidence to substantiate for a lack of supervision because Administrator #1 emphasized that at no time was Child #1 left alone or unsupervised. Rather, supervision was transferred from the classroom staff to the administrator to ensure both the child's safety and the timely evacuation of the remaining children.

On August 14, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding positive behavior supports, unqualified staff, and not meeting the staffing ratio. An investigation was conducted on August 25, 2025. An investigation was conducted on October 24, 2024. The complaint is substantiated for infants and non-walking one-year-old children sleeping on cots because staff stated they begin transitioning infants to cots at eleven months old. Staff #1 stated sometimes the children can walk and sometimes they cannot. The complaint is substantiated for infants not being held to feed and mixing formula with cow s milk because staff stated on occasion, she places an infant in a bouncer seat and holds the bottle to feed the infant and they begin gradually transitioning infants to cow s milk by mixing it with the infant s formula with parent permission. There is no evidence to substantiate for improper staff-to-child ratio because the classrooms and reviewed attendance records were observed to be in proper staff-to-child ratio. There is no evidence to substantiate for playground capacity because staff denied the allegations and the three separate outdoor play areas measure for more children than there are enrolled in each age group. There is no evidence to substantiate for infant sleep checks because Staff #1 stated she conducts them every 30 minutes or more frequently if the child seems congested or hit their head that day and sleep checks were documented in Brightwheel during the visit. There is no evidence to substantiate for meals and snacks, diapering, handwashing, open-door policy, and visitors because staff denied the allegations.

There is no evidence to substantiate the allegations of not following diapering requirements and not following screentime requirements because interviewed staff stated diapers are checked hourly and there is little to no screentime given to the children.

A picture of a sleeping infant, wrapped in a swaddle, in a bouncy seat was observed on the Brightwheel app. Staff #3 stated the picture was taken prior to the infant being moved to the crib. Based on the picture, it appeared the infant was placed in the bouncy seat to put the infant to sleep. There is no evidence to substantiate for inaccurate attendance records, dishonesty, and improper discipline because staff denied the allegations and the attendance records for September 21, 2022, and September 22, 2022, did not appear to be altered or forged .
