Child care staff shall supervise every child in care at all times.
REPEAT CITATION, PREVIOUSLY CITED ON AUGUST 5, 2025. The program self-reported that a two-and-a-half-year-old child was left inside the program on August 22, 2025, for approximately 3 minutes. Staff A stated that they went inside from the playground with three children to have them wash their hands and then returned to the playground. Staff A did not notice that while inside, one child left the area and went around a corner into a separate section of the classroom. Staff A confirmed that they returned to the playground with only two children, leaving one child inside inadvertently. The licensing coordinator’s review of the video footage confirmed that the child was inside without supervision for approximately three minutes. This constitutes one repeat citation and failure to comply with one approved Corrective Action Plan regarding supervision of children. Corrective action: On 8/25/25, a new policy was implemented at the program to ensure active supervision of children at all times when transporting individuals or small groups of children inside and back outside. All studios have been equipped with a "small group safety check" on their clipboard. When children move inside, the educators will log on the sheet who they have brought inside with them and upon returning back outside, will check that all children brought inside, made it back out successfully. This will ensure educators know where each child is at all times. Children will be accompanied by an educator at all times. Additionally, children are not transported in groups larger than 3 when going inside to use the potty, wash hands etc. Educators will also complete name to face recognition of all children upon return after leaving the playground for any reason - particularly during pickup times.
