The licensing coordinator reviewed staff attendance records and found one recorded who had not been determined eligible by the Unit. The director confirmed those individuals were in attendance as indicated. Corrective action: this was corrected on October 9th by completing fingerprinting which we had already done but we missed logging it correctly on this site.Has been corrected and the staff person was not working with students due to an injury in that time period.

Child care staff shall not allow children younger than 3 years of age to have access to toys, toy parts, and other materials that pose a suffocation or choking risk or are small enough to be swallowed, including, but not limited to, coins, balloons, exposed foam padding, or empty plastic bags.

The licensing coordinator observed several small parts that were within reach of children under three years old. The licensing coordinator observed that a staff member was present at the small parts activity, but stepped away several times to assist other children, leaving the children under the age of three years with the small parts. Corrective action: we have relegated a staff member to that area which we had moved this fall when we redesigned the classroom. We no longer have any students under 3 in our program but will bet conscious going forward 10/8/25

Prior to employment or residency, as applicable, and every 5 years thereafter, all child care staff, substitutes, other employees, and volunteers who may be alone with children or are included in staff to child ratios, and household members age 10 years or older, shall submit for a background record check using NH Connections.

The licensing coordinator observed one staff in the program who had not been determined eligible by the Unit. A review of department records confirmed the staff had a gap greater than 180 days and did not submit for a background record check prior to employment. Corrective action: The staff member's background check had lapsed, this will be corrected on Feb 14th. The staff has spent interim days visiting other programs for training purposes and working in summer inventory. Future staff will not be permitted to work with children prior to completion of this process. We have scheduled meetings to research licensing standards to be sure we are not overlooking future new hire regulations. I acknowledge the directed CAP provided and will not allow staff who are not eligible to be present in the program.

All center directors, agency administrators, site coordinators, or site directors, and all other child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall keep on file documentation of completion of a minimum of 6 hours of professional development, which shall be completed in accordance with the following: (1)  Within 90 days of the first date of employment; (2) Within 2 weeks for programs operating 3 months of the year or less; or (3) By providing documentation of previous completion.

The licensing coordinator’s review of three staff files revealed that one staff employed by the program for more than 90 days had not completed the required health and safety trainings. Corrective action: The staff member has been signed in to Prosolutions to complete the 12 credit hours that we thought were mandatory but were unable to access. The coordinator helped us, and she has only 4 credit hours to complete to be compliant. These will be completed by Feb 25th 2024 She has agreed to a schedule of completing 2 trainings per month, and to maintain her 12 credits, as well as working toward her assistant teacher status. We are monitoring all training hours for each staff member bimonthly The staff member is not unsupervised with children and works with a certified teacher at all times. Her background and experience, as well as experience in private homes with some of our families over the past five years has been exemplary and given us confidence in her of her ability to be safe and appropriated around the children she is with during the mornings she is with two trained and experienced teachers.
