Programs shall comply with approved corrective action plans and corrective action plans issued in accordance with (i) and (j) above.

This Statement of Findings includes repeat a citation for He-C 4002.09(a), which constitutes failure to comply with one approved corrective action plan for a repeat citation, as noted herein. Corrective action: We have created a spreadsheet with each statement of findings that will be reviewed on a regular basis to ensure extra supervision and compliance. We will review the spreadsheet at regular meetings to ensure that we are keeping on top of citations and they do not happen again.

He-C 4002.06(m) · Correction status not published

All center directors, agency administrators, site coordinators, or site directors, and all other child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall keep on file documentation of completion of a minimum of 6 hours of professional development, which shall be completed in accordance with the following: (1)  Within 90 days of the first date of employment; (2) Within 2 weeks for programs operating 3 months of the year or less; or (3) By providing documentation of previous completion.

The licensing coordinator's review of two staff files revealed that one staff, with a hire date in April 2023, had not completed the required health and safety training within 90 days of hire. Corrective action: We will address this requirement at our upcoming staff meeting on 1/24 and our registrar will send reminders to staff when they are nearing the end of their 90-day window to ensure they are aware of the required trainings. If staff do not complete this training, they will be required to do so before re-entering program.

He-C 4002.33(a) · Correction status not published

The program shall maintain on file for each staff member: (1) Documentation of the staff’s qualifications, including a resume, transcripts, diplomas, or any other documentation, which demonstrates their qualifications; (2) Documentation of a completed background check in accordance with He-C 4002.41; (3) A record of health screening, in accordance with (b) below; (4) Documentation of orientation, certifications, training, and professional development, as applicable; (5) Documentation verifying the staff reviewed the program’s emergency operations plan in accordance with He-C 4002.16(j); (6) Documentation verifying the staff reviewed their job description required in He-C 4002.16(e); and (7) Documentation of supervision regarding disciplinary actions or investigations specific to the staff member.
