In the Infant classroom, the freezer used to store breast milk was not equipped with a food service thermometer. Staff added a thermometer to the freezer. Corrective action: Breastmilk was moved to a traditional freezer for optimum temperature. And thermometer was placed in infant freezer during visit. When replacing a fridge/freezer we will make sure thermometers are added. And notes have been added on each fridge as a reminder to have thermometers inside.
In the Infant Classroom, the licensing coordinator observed an infant in a crib wearing a Love to Dream Swaddle Wrap. The staff confirmed the infant was 4 months of age. Corrective action: Parent was notified this was not acceptable. We have changed it out to a traditional sleep sack. Will update our "Infant Supply List" we send to parents. We will not allow children older than 3 months to be swaddled or placed in restrictive or weighted sleep suits or devices unless there are written medical orders from the child’s health practitioner.
This Statement of Findings includes repeat citations for He-C 4002.41(a) which constitutes one repeat citation and failure to comply with 2 approved corrective action plans, as noted herein. Corrective action: As directors, we have been relying on the documents created as a result of registering a staff member in the system. Minors do not trigger the generation of this in the NH Connection system, as it does for others, which has created confusion. We will maintain hard copies of a Household and Personnel form when hiring minors, as they are not automatically generated in the NH Connections system and submit a hard copy separately in the future. We recognize that there are multiple citations for this as a center, but there have been many changes within NH Connections within this timeframe, which has lead to confusion and miscommunication. As a center, we have followed our corrective action plans as we have stated, but state systems have been changing at the same time, which requires additional follow up on our internal processes. We have added this to sent to all employees in the onboarding process and have requested a completed hard copy to be returned. This duplicative effort between us and the NH Connections should help to eliminate any potential oversite for this document.
A review of department records revealed that one staff under the age of 18 years did not submit a household and personnel form prior to employment. The director confirmed that the individual did not submit a household and personnel form prior to employment. The program was previously cited for this non-compliance under He-C 4002.04(l)(1) following a monitoring visit on February 11, 2022 and submitted a corrective action plan, which stated, in part "The assistant director has also been added as a notary to ensure timely completion of forms. We have also added another notation space for onboarding documents required in Gusto, our payroll and onboarding software system. We are hoping the new electronic systems will help steam line these processed." A previous corrective action plan for He- C 4002.04(l)(1) following a monitoring visit on March 19, 2021, stated, in part, "We have also added a tracking category on our employee management software to ensure this is completed." Corrective action: As directors, we have been relying on the documents created in NH Connections as a result of registering a staff member in the system. Minors do not trigger the generation of this in the NH Connection system, as it does for others, which has created confusion. We will maintain hard copies of a Household and Personnel form when hiring minors, as they are not automatically generated in the NH Connections system and submit a hard copy separately in the future. Additionally all new hires will receive this document in the onboarding process and are required to complete and hand deliver to the director. We recognize that there are multiple citations for this as a center, but there have been many changes within NH Connections within this timeframe, which has lead to confusion and miscommunication. Any staff not deemed eligible by the Unit will not be on the premises of the program until they are deemed eligible. The H and P was delivered notarized and signed to CCLU in less than 24 hours from the time of the citation.
