The program shall maintain daily child attendance records, in either paper or electronic format, including the child’s full name, date of birth, and their arrival and departure times in the program and each room or group throughout the day, as applicable, recorded in real time by staff or parents.

The licensing coordinator's review of the daily attendance revealed that there were five staff present and no staff were signed in. Corrective action: Staff have been retrained so they know that the need to clock in as well as sign in each day to get paid and show they are working. Moving forward all staff will be clocking in and signing in on the tablets to show that they are in attendance.

Authorized staff shall complete training in medication safety and administration every 3 years.

The licensing coordinator's review of three staff files revealed that one staff who administers medication had not completed the training in medication safety and administration within the past three years. Corrective action: The Staff member has completed and renewed his administering medication on 01/24/2024. Moving forward Files are to be checked bi-weekly to make sure that all trainings for all staff are current.

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.

The program shall request and maintain on file for each child documentation of immunizations in accordance with RSA 141-C:20-a, RSA 141-C:20-b, and He-P 301.14.

The licensing coordinator's review of nine children's records revealed there was no record of immunization on file for two children on their first day of attendance, and no record of physical examination on file for one child who has been enrolled and attending for more than 60 days. Corrective action: Camp Doc's was reviewed 12/08/23 and all children that were not at 100% were told they could not return to program until they are at 100%. Moving forward Children are to not start in any program unless they are at 100% in camp docs. Camp doc's is to be reviewed weekly to make sure all children stay current.

The program shall maintain on file for each staff member: (1) Documentation of the staff’s qualifications, including a resume, transcripts, diplomas, or any other documentation, which demonstrates their qualifications; (2) Documentation of a completed background check in accordance with He-C 4002.41; (3) A record of health screening, in accordance with (b) below; (4) Documentation of orientation, certifications, training, and professional development, as applicable; (5) Documentation verifying the staff reviewed the program’s emergency operations plan in accordance with He-C 4002.16(j); (6) Documentation verifying the staff reviewed their job description required in He-C 4002.16(e); and (7) Documentation of supervision regarding disciplinary actions or investigations specific to the staff member.

The licensing coordinator's review of three staff files revealed that the files did not contain documentation verifying the three staff had reviewed the program’s emergency operations plan. The program was previously cited for noncompliance with this rule following a September 26, 2022 visit. A corrective action plan submitted by the program and approved by the department stated, "The School's Out Program Director ensured that all staff completed, reviewed, and signed the Emergency Operations Plan. To help prevent this from happening again, an orientation/on-boarding checklist has been developed. This will be used to compete the on-boarding process as well as to sure the files periodically to be sure all required paperwork is present." Corrective action: All staff have been retrained on both the associations EOP plan and the schools EOP plan 12/18/2023. They all have a copy of the signed form that they completed the review in their PD folder. Moving forward each location has been given a monthly calendar with all drills listed that will be turned in to management at the beginning of the school year so that Management can follow up on days trainings are to be completed. Quarterly, Program directors will be holding an all staff meeting where the EOP review will be done as a large group. and signed off by the Director.
