The licensing coordinator reviewed program records and determined that one staff member did not submit for a background record check prior to employment, and that one staff member under the age of 18 years did not submit a household and personnel form prior to employment. The director confirmed those individuals have been present and working at the program. The program was previously cited for non-compliance with this rule following a July 28, 2021 visit. The licensing coordinator's observation of a staff's purse on the floor of an unlocked closet in the Moose Mountain classroom. Corrective action: Staff purse was put up on top shelf where it will be stored now and in the future.

Programs shall maintain bathroom facilities in accordance with the following: (1) Sinks, toilets, footstools, potty chairs, and adapters shall be cleaned and sanitized at least once a day and when visibly soiled; (2) Bathroom floors and other surfaces adjacent to toilets, including but not limited to walls, shall be cleaned and sanitized at least weekly, and when visibly soiled; (3) Toilet paper, individual cloth or paper towels, and liquid soap from a dispenser shall be available and accessible to children and staff; and (4) Bathrooms shall have a functional means of outside ventilation.

The licensing coordinator's observation that there was no toilet paper available and accessible to the children in the Wildcat Mountain bathroom. Corrective action: Toilet paper has been put in the bathroom. The roll will be replaced now and in the future.

The licensing coordinator's review of the daily child and staff attendance records revealed that there were three children signed into the Mount Blue Job classroom and only two children present, and there were three staff signed into the Mount Washington classroom when only two were present. Corrective action: Staff will make sure that they are correctly documenting when a child or themselves leave a room and sign in/out now and in the future. All staff are being re-trained on signing in and out of classrooms. The retraining will prevent this non compliance now and in the future.
